介入放射学诊疗中患者所受辐射剂量调查研究 被引量:8

INVESTIGATION OF PATIENT RADIATION DOSES IN INTERVENTING RADIOLOGY PROCEDURES

摘要 本文调查了在某医院接受介入放射学诊疗的405例患者的人口统计学和辐射剂量数据信息。研究发现,同一介入操作的不同患者在剂量数据上有较大的差异;除个别部位外,同一部位的不同介入操作类型之间在剂量和相关数据上有显著性差异;而不同部位介入操作间在剂量和相关数据上多数具有显著性差异。20%的患者(79/405),主要是脑血管、肝脏、盆腔介入操作等的累积剂量CD(cumulativedose)超过1 Gy。各种栓塞操作、经颈静脉肝内门体静脉分流术和经皮经肝胆管引流术等是和患者的高辐射剂量相联系的。CD和剂量-面积乘积DAP(dose-area-product)的总体相关性较好(r=0.92,p<0.01);摄影CD与摄影DAP、透视CD与透视DAP均显示较好的相关性(r分别为0.95、0.88,p均<0.01)。由摄影帧数和透视时间分别估算摄影CD(或DAP)与透视CD(或DAP),再将两者结合预测总CD(或总DAP)比较精确。 An observational study was performed at Department of Clinical Medicine of an university Demographic inforation and radiation dose data were collected of 405 cases undergoing specific interventing radiology procedures. It was observed that doses varied among eases of the same procedure. Except for individual lountion, statistically significant differences in one or more of the dose data were observed among most of subgroups in the procedures for the same organ. There were also statistically significant differences in the dose data among most of the procedures for different organs. In 20% of cases (79 of 405), principally brain vas, liver, pelvis cavity procedures, their cumulative dose (CD) was greater than 1 Gy. Embolization procedures, transjugular intrahepatic portosystemic shunt (TIPS) creation, and percutaneous transhepatic cholangio drainage (PTCD) are related with the high doses of patient. It was observed that, there were correlation between good CD and dose-area-product (DAP) ( r = 0.92, p 〈 0.01 ), as well as that the good correlation between CD and DAP of fluorographic settings ( r = 0.95, p 〈 0.01 ), and CD and DAP of fluoroscoping ( r = 0.88, p 〈 0.01). Is is more accurate to estimate CD(or DAP) of photographic settings and CD (or DAP) of fluoroscoping settings respectively based on number of images and fluoroscopy time and then to estimate total CD (or DAP).
