摘要
目的研究上皮性卵巢癌组织中雌激素受体(estrogen receptor,ER)、孕激素受体(progesterone receptor,PR)、血管内皮生长因子(vascu lar endothe lia l grow th factor,VEGF)及肿瘤内微血管密度(m icrovesse l dens ity,M VD)的表达及相关性,探讨其表达与病理生理学特征的关系。方法采用免疫组化方法检测ER、PR、VEGF在12例正常卵巢、21例良性上皮性卵巢肿瘤和46例上皮性卵巢癌中的表达。人工计数M VD,分析VEGF与ER、PR是否存在相关性。结果恶性卵巢上皮性肿瘤ER、PR阳性表达Ⅰ、Ⅱ期高于Ⅲ、Ⅳ期(P<0.05),高、中、低分化之间的ER、PR、VEGF表达的差异无统计学意义(P>0.05),恶性肿瘤组VEGF阳性率为63.04%,良性肿瘤组VEGF阳性率为33.33%,两组比较,差异有统计学意义(P<0.01);恶性肿瘤组中M VD明显高于良性肿瘤组及对照组(P<0.01);恶性肿瘤组中ER、PR表达均与VEGF表达有相关性(P<0.05)。结论卵巢恶性肿瘤的ER、PR阳性率随临床分期进展而降低,提示随着恶性程度的增加,其对内分泌治疗的反应可能降低;ER、PR可能通过刺激VEGF表达而促进卵巢上皮性癌组织中微血管生成,进而促使肿瘤的转移。
Objective To investigate the expression and correlation of estrogen receptor (ER),progesterone receptor (PR) and vascular endothelial growth factor(VEGF) in ovarian tumors. Methods The expression of ER,PR and VEGF in 12 cases of normal ovaries,21 cases of benign ovarian tumors and 46 cases of epithelial ovarian cancers were detected by streptavidin peroxidase (SP) immunohistochemistry method, and MVD (microvessel density) level was measured. Results The expression of ER and PR in stage Ⅰ and stage Ⅱ was higher than stage Ⅲ and stage Ⅳ of ovarian epithelial cancers (P〈0. 05). The expression of ER, PR and VEGF had no statistical significance among different histological grades of the malignant tumors. The expression of VEGF in malignant tumors was higher than benign ones (P〈0. 05). The expression of ER and PR was correlated with that of VEGF in malignant tumors. Conclusion The expression of ER and PR in ovarian epithelial cancers is decreased with the clinical staging, suggesting that the reaction to treatment would retard. ER and PR may influence the angiogenesis of ovarian cancer through promoting the expression of VEGF and further prompt the metastasis of ovarian cancer.
