摘要
目的：为临床上常见的胫骨骨不连和骨缺损提供一种新的术式。方法：在５４侧成人下肢标本上，观测了腓浅血管和腓血管穿支的走行、分布和吻合。通过解剖学研究，我们设计、创用了以腓浅血管筋膜为蒂的胫、腓骨骨膜瓣转位术，结果：经临床应用８例（胫骨中段骨不连５例、骨缺损２例、胫骨上段骨缺损１例）均取得良好疗效。结论：该骨膜瓣具有位置表浅、解剖容易、血管蒂长、手术安全可靠等优点，适于修复胫骨上、中段骨不连和小范围骨缺损。
Objective:This article provides a new operative method for repairing tibial non- union or bone defect.Methods:The route,distribution and anastomoses of superficial peroneal vessel and perforating branch of peroneal artery on 54 adult leg specimens。Based on the anatomic study,the transposition of distal tibiofibular periosteal flap pedicled with fascia superficial peroneal vessels was de- signed.Results:This new operation has been successfully used in 8 cases(non-union of 5 cases,bone defect of 2 cases in middle tibial and bone defect of 1 case in proximal tibial).Conclusions:The vascu- lar pedicle of the periosteal flap is relatively long and anatomical location is superficial and constant. The operation is secure and adapt to repair non-union or small limits bone defect in proxlmal and mid- dle segments of the tibial.
