脑梗死患者血浆同型半胱氨酸变化及叶酸的干预作用 被引量:7

摘要 目的:了解同型半胱氨酸(homocysteine,Hcy)、叶酸水平与脑梗死的关系,观察叶酸干预治疗高Hcy血症的疗效。方法:脑梗死组患者26例以及非脑梗死对照组患者18例,用酶免分析法检测血浆Hcy水平,放免法测定叶酸浓度。脑梗死组中高Hcy血症患者给予叶酸5mg,每日3次,连续1周,观察服药前后血浆Hcy水平变化。结果:脑梗死组患者血浆Hcy浓度明显高于对照组,叶酸浓度低于对照组,而且伴有颈动脉斑块患者的Hcy浓度高于无颈动脉斑块者;高Hcy血症的脑梗死患者用叶酸治疗1周,血浆Hcy浓度平均下降4μmol/L。结论:脑梗死患者血浆Hcy水平明显升高,叶酸干预治疗能使高Hcy血症脑梗死患者血浆Hcy浓度明显下降。 Objective To study the relationship between the plasma levels of homocysteine (Hey) and folic acid and cerebral infarcts and to explore the efficacy of interventional therapy of folio acid for patients with hyperhomocysteine. Methods 26 patients with cerebral infarcts were assigned to the case group and 18 patients without cerebral infarcts were assigned to the control group. Their plasma Hey levels were measured by using enzyme immunoassay (EIA) and plasma folic acid levels were measured by radioimmunoassay (RIA). The patients with cerebral infarcts and hypcrhomocysteinerrta were treated daily with 15mg of folic acid for one week, then their plasma Hey levels were detected again. Results The plasma Hey level was significandy higher in the patients with cerebral infarcts than in the controls, with (15.74 ± 4. 50) μmol/L vs (10. 65 ± 3.29 ) μmol/L, The plasma folio acid level was significantly lower in the patients with cerebral infarcts than in the controls. The Hey level in the patients with cerebral infarcts and carotid artery plaque was significantly higher than in the patients without carotid artery plaque, with (18.56±3.51)μmol/L vs (13.32±2. 71)μmol/L. The Hey level of patients with cerebral infarcts and hyperbemocysteinemia was decreased to a level of 4 μmol/L after one week of administration with folie acid. Conclusions Hey level is markedly elevated in the patients with cerebral infarcts. Folic acid can dramatically lower plasma Hey level in the patients with cerebral infarcts and hyperhomoeysteine.
