摘要
目的:探讨MRI动态增强扫描(DMRI)在丰满垂体中的鉴别诊断价值。材料和方法:选取我院1999年10月一2005年4月期间具备:①垂体高径在男性超过8mm,女性超过10mm;②垂体双侧基本对称;③MRI平扫及普通增强扫描(CMRI)中垂体信号均匀,并完成DMRI检查;④有完整的临床资料为人选条件的丰满垂体共22例,回顾性分析其MRI图像资料。结果:非瘤性丰满垂体19例,其中青春期12例,流产后4例,产后l例,增强后24。48s显示以垂体结节部为中心向远侧及两侧呈渐进性、对称性弥散样强化,增强后60—72s垂体均匀强化;垂体增生2例,增强后24—72s见以垂体结节部为中心向远侧及两侧呈渐进性、对称性弥散样强化,但结节部强化程度明显高于垂体远侧部。瘤性垂体3例,均为垂体微腺瘤。
To explore the value of differential diagnosis using dynamic contrast - enhanced MR imaging (DMRI) in plumpy pituitary gland. Materials and Methods- MRI manifestations of 22 cases with plttmpy pituitary gland were analyzed retrospectively from October 1999 to April 2005, the inclusion criteria were:① more than 8 nun in the height of pituitary gland in male and more than 10 mm in female.②no significant differences in shape between the right and left side of pituitary stalk.③homogeneous signal intensity of pituitary gland in pre - contrast and conventional contrast MR imaging. ④with integrated clinical data. Results:Of the 22 cases, 19 cases were non - tumorous plumpy pituitary gland, of which 12 cases were youthhood, 4 cases were post - abortion and one case was postpartum, the earliest contrast - enhanced structure was the pars teberalis, followed by gradual and symmetrical enhancement from pars teberalis to pars distalis and the both side of pituitary stalk at 24 -48 seconds, homogeneous signal intensity was seen at 60 - 72 seconds after administration of gadopentetate dimeglumine, 2 cases were hyperplasia of pituitary gland, the enhanced pattern was similar to the former group, but the duration of enhancement deferred at 24 -72 seconds, and signal intensity of pars teberalis was obviously higher than of pars distalis. 3 cases were pituitary microadenoma, nodules with low signal intensity were clearly demonstrated at 36- 72 seconds after administration of gadopentetate dimeglumine.Conclusion: DMRI appears to have great potentials in differentiating the tumorous and non - tumorous plumpy pituitary gland.
