摘要
目的:观察加味四君颗粒、贞芪扶正颗粒联合化疗治疗胃癌的临床疗效。方法:120例胃癌术后患者按数字化随机法分为加味四君组(加味四君颗粒加化疗)、贞芪扶正组(贞芪扶正颗粒加化疗)、对照组(单纯化疗),每组40例。观察治疗前后卡氏评分、外周血象和血液动力学变化,并观察自然杀伤(NK)细胞、T淋巴细胞亚群、可溶性白介素2受体(sIL-2R)等指标的变化。结果:加味四君组和贞芪扶正组胃癌患者化疗后生存质量明显改善,外周血白细胞和红细胞数稳定,单纯化疗组外周血白细胞、红细胞、血红蛋白浓度均较化疗前明显下降;加味四君组和贞芪扶正组患者化疗后NK细胞活性、CD3、CD4T细胞数和CD4/CD8比值升高,血清sIL-2R的浓度明显下降;单纯化疗组化疗后患者NK细胞活性、CD4T细胞数和CD4/CD8比值下降,血清sIL-2R的浓度升高而全血黏度无明显改变。结论:加味四君颗粒、贞芪扶正颗粒对化疗有增效减毒作用,联合化疗对胃癌术后患者机体免疫功能有保护和促进作用,并能防治化疗引起的骨髓抑制。
Objective： To observe clinical therapeutic effects of Jiawei Sijun Granules and Zhenqi Fuzheng Granules combined with chemotherapy on gastric cancer. Methods：One hundred and twenty cases of gastric cancer after operation were randomly divided into 3 groups, a Jiawei Sijun group （Jiawei Sijun Granules combined with chemotherapy）, a Zhenqi Fuzheng group （Zhengqi Fuzheng Granules eornbined with chemotherapy） and a control group （simple chemotherapy）, 40 eases in each group. Changes of Karnofsky score, peripheral blood and hemorrheologic indexes, NK cell activity, T-lyrnphoeyte subgroup and serum soluble interleukin-2 receptor （sIL-2R） before and after treatment were determined. Results： After chemotherapy, quality of life improved, and peripheral blood leukocyte and erythroeyte eoums in both Jiawei Sijun group and Zhenqi Fuzheng group were stable. Peripheral blood leukocyte, erythrocyte and hemoglobin content in the control group decreased significantly. NK cell activity, CD3 and CD4 T-lymphocytes and the ratio of CD4/CD8 increased significantly, and serum sIL-2R content decreased significantly in Jiawei Sijun group and Zhenqi Fuzheng group. NK cell aetivity,CD4 T-lymphocyte count and the ratio of CD4/CD8 increased significantly and serum sIL-2R level increased in the control group. Conclusion： Jiawei Sijun Granules and Zhenqi Fuzheng Granules can increase the therapeutic effect and reduce side effects caused by chemotherapy ,and their combination with chemotherapy can protect immune functions of gastric cancer patient after operation and prevent bone marrow suppression induced by chemotherapy.
