急性心肌梗塞患者血浆内阿片肽和精氨酸加压素含量及其临床意义 被引量:9

摘要 采用放射免疫方法测定了正常人、急性心肌梗塞和不稳定型心绞痛患者血浆β内啡肽、强啡肽和精氨酸加压素(AVP)的浓度。结果:血浆β内啡肽、强啡肽和AVP浓度在AMI、不稳定型心绞痛患者较正常人明显增高(P<0.001,P<0.01),而在AMI组与不稳定型心绞痛组3种激素浓度均无显著性差异(P均>0.05)。AMI组9例有严重并发症者与13例无并发症者相比,有并发症者3种激素均显著高于无并发症者(P均<0.05)。提示:缺血性疼痛是一个强的应激原,导致这些激素释放增加,另外,内阿片肽和AVP在缺血性疼痛刺激下平行升高可能是使机体在应激状态时保持内环境的相对稳定。 Radioimmunoassay was applied to determine the plasma levels of βendorphin (βEP),dynorphin(DP),arginin vasopressin (AVP) in patients with acute myocardial infarction(AMI),patients with unstable angina pectoris (AP),and normal persons.The results showed that the plasma levels of βEP,DP,AVP were significantly higher in both AMI and unstable AP groups than those in the normal control group ( P <0 001, P <0 01),while the plasma levels of the three hormones in AMI and unstalbe AP groups had no significant differences statistically ( P >0 05).In AMI group,there were 9 patients with severe complications and 13 patients without any complications.The plasma levels of the three hormones in the AMI group with complications were markedly higher than those in the AMI group without any complications( P <0 05).It is suggested that ischemic pain is a strong stressor leading to the increase of the release of the hormones.In addition,under the stimulation of ischemic pain or stress status,the parallel increases of plasma levels of endogenous opioid peptides(EOP) and AVP maintain the internal environment of the body relatively stable,which is possibly one of the adaptive compensatory mechanisms developed in the longterm evolutionary process of the body.
