摘要
目的:探讨进展期胃癌根治术后复发的早期诊断及治疗的相关问题。方法:回顾性分析我院1997年12月至2004年12月收治的106例进展期胃癌根治术后复发病例,按治疗方法进行分组,采用Kaplan-Meier方法绘制生存曲线,进行累积生存率的比较。结果:全组106例患者平均复发时间为术后18.3个月,2年内复发占81.1%。复发癌根治性切除组17例,姑息性切除组19例,非手术切除组21例。3组中位生存时间分别为≥24.0、9.0和3.0个月,两两之间存在显著差异(P<0.05)。非手术治疗组49例,中位生存时间2.0个月,与手术各组相比,存在显著性差异(P<0.05)。结论:进展期胃癌术后复发患者预后较差,早期发现和积极治疗有助于延长患者生存时间。
Objective： To study the related questions of early diagnosis and treatment for postoperative recurrence of advanced gastric cancer. Methods： The clinical data and survival rate of 106 cases with postoperative recurrence of advanced gastric cancer, from Dec 1997 to Dec 2004, were retrospectively analyzed. Kaplan-Meier was used to depict the survival curves and to compare the differences among the cumulative survival rates according to varied treatments. Results： The mean recurrence time was 18.3 months. 81.1% of postoperative recurrence was found within 2 years. Radical resection, palliative resection and bypass process were carried out in 17 patients, 19 patients and 21 patients, for which the median survival time was ≥ 24.0, 9.0, and 3.0 months, respectively. The difference among them was significant （P〈0.05）. The median survival time was 2 months for the 49 patients with supportive treatment, and it was remarkably lower than those of the surgical operation groups （P〈0.05）. Conclusions： The prognosis of recurrent gastric cancer is poor. Early diagnosis and active treatment can prolong survival time of the patients.
