摘要
目的探讨单纯及复合小梁切除术的术后近期并发症,为术式选择和并发症的处理提供依据。方法2003年1月至2004年12月在我中心行青光眼小梁切除术的全部住院患者共321人(400眼)。术式包括单纯小梁切除术,小梁切除联合羊膜植入术,小梁切除联合丝裂霉素C(MMC),小梁切除联合MMC,羊膜植入术,小梁切除术联合黏弹剂留置。记录各种术式近期并发症及处理情况。结果对小梁切除术后近期并发症118眼采用保守治疗,治愈111眼(占94.07%)。MMC组浅前房的发生率低于羊膜组,但与单纯组无明显差异;浅前房合并脉络膜脱离的发生率低于羊膜组,与单纯组无明显差异;前房积血的发生率与羊膜组、单纯组均无明显差异。结论小梁切除术后近期并发症的保守治疗效果显著。根据患者的具体情况和术者的技术水平选择手术方法,可降低手术风险,减少并发症,取得较好效果。
Objective To review the early complications of trabeculectomy and the efficiency of the treatment from a series of glaucoma patients. Methods From January 2003 to December 2004, all patients in Peking University Eye Center who performed trabeculectomy were reviewed .The procedures include trabeculectomy （112 eyes）,trabeculectomy with human amnion transplantation （118 eyes）, trabeculectomy with miotomycin C （MMC） （143 eyes）,trabeculectomy with beth human amniotic membrane and MMC （24 eyes）, trabeculectomy with viscoat （3 eyes） .The early complications were described and the mechanisms were analyzed. The treatments were also discussed. Results Most of the early complications （ 111 eyes,94.1% ） were treated by medicines. There was significant different between trabeculectomy with human amnion and trabeculectomy with MMC of some early complications. Conclusions Most of the early complications may be treated by medicine. Choose different surgical procedures by individual difference may get better approach.
