摘要
目的:根据近年来对瘢痕疙瘩和增生性瘢痕治疗研究的进展,探讨其可行的治疗方法。资料来源:应用计算机检索Medline1980-01/2005-01相关瘢痕疙瘩和增生性瘢痕治疗的文献,检索词为“keloid,hypertrophicscar,treatment”,并限定文献语言种类为English.同时计算机检索CBM1980-01/2005-01相关瘢痕疙瘩和增生性瘢痕治疗的文献,检索词为“瘢痕疙瘩,增生性瘢痕,治疗”,并限定语言种类为中文。资料选择:对资料进行初审,开始查找全文。纳入标准:①相关瘢痕疙瘩治疗方法。②相关增生性瘢痕治疗方法。排除标准:综述文献、重复研究、Meta分析类文章。资料提炼:共收集到42篇关于瘢痕疙瘩和增生性瘢痕治疗方法方面的研究,纳入24篇符合标准的文献。资料综合:瘢痕疙瘩和增生性瘢痕形成的机制尚未明了,目前所知与外伤,烧伤,手术,感染,异物,机体局部胶原代谢失调,遗传及局部免疫功能等多种因素有关。故治疗方法多种多样。瘢痕治疗虽然类型与方法很多,但是尚无一种方法能达到完全治愈的效果。结论:增生性瘢痕治疗水平的提高有赖于人们对其形成机制的认识,相信随着分子生物学技术的发展,瘢痕形成机制在细胞、分子水平的进一步阐明,希望在不久的将来会彻底了解增生性瘢痕和瘢痕疙瘩形成的机制。
OBJECTIVE: To explore the feasible therapeutic way according to the development of cheloid and hypertrophic scar in recent years.
DATA SOURCES: The literatures on keloid and hypertrophic scar treatment published between January 1980 and January 2005 were collected from the Medline database by using the word of "keloid, hypertrophic scar and treatment" in English. Moreover, the literatures published from January 1980 to January 2005 were collected from the CBM by using the word of "keloid ; hypertrophic scar and treatment" in Chinese.
STUDY SELECTION: The literatures were selected firstly and found the complete papers. Inclusive criteria: ①Thcrapeutic way of keloids. ②Therapeutic way of hyperplastic scar. Exclusive criteria: review, repeated study and Meta analytical papers.
DATA EXTRACTION: The 42 literatures of the treatment for keloids and hypertrophic scars were 24 papers that were included.
DATA SYNTHESIS: The exact pathological mechanism underlying keloids and hypertrophic scars was unknown. At present it was related with multiple factors, such as: external injury, burn, operation, infection, foreign body, local collagen dis-metabolism, heredity and local immune function and so on. Thus, the therapeutic way was various. The methods of the treatment of keloids and hypertrophic sears were many, but none of them had the effect with complete cure.
CONCLUSION: The increase of therapeutic level of hyperplastic scar is dependent on the cognition of its forming mechanism. With the improvement of molecular biology and the further clarification of cellular and molecular level of cicatrisation, it is hope to comprehend fully the mechanism of hyperplastic scar and keloid.
