摘要
目的评价左旋氨氯地平与厄贝沙坦在高血压病患者早期肾保护作用方面的疗效。方法入选高血压病1级、2级患者71例,随机分为两组:左旋氨氯地平组为35例,口服左旋氨氯地平2.5 mg/d;厄贝沙坦组为36例,口服厄贝沙坦150 mg/d。观察12个月,治疗前、后分别检测肾小球滤过率(GFR)、血、尿β2微球蛋白(β2-MG)及血压。结果①左旋氨氯地平组和厄贝沙坦组在治疗后GFR均升高;厄贝沙坦组的疗效优于左旋氨氯地平组(P<0.01)。②左旋氨氯地平组和厄贝沙坦组在治疗后对血、尿β2-MG的降低差异有统计学意义,厄贝沙坦组的疗效优于左旋氨氯地平组(P<0.05)。③两组在用药后收缩压、舒张压均达到目标值(P<0.01)。两组差异无统计学意义。结论左旋氨氯地平和厄贝沙坦在有效降压的同时,都能不同程度使血、尿β2-MG降低,使GFR升高;厄贝沙坦的作用优于左旋氨氯地平。它们对高血压病早期肾功能有保护作用。
Objective To investigate the protective effects of levoamlodipine and irbesartan on kidney of patients with early essential hypertension. Methods There were 71 patients with hypertension grades 1 or 2 allocated randtxnly to 2 groups,one group received levoamlodipine 2.5 nag （n = 35） and mother irbesartan 150 nag （n = 36） once a day. The GFR,blood and urinary microglobulin （β2-MG） and blood pressure were measured before and after treatment, respectively. Results OGFR increased significantly after treatment in both groups. The effect of irbesartan was more significant than that of levoamlodipine （ P 〈 0.05）. （2） Blood and urinary β2-microglobulin decreased significantly after treatment with levoarnlodipine and irbesartan. The effect of irbesartan in decreasing blood and urinary k-urinary microglobulin was more significant than that of levoamlodipine （P〈0.05）. （3）Target blood preasure was achieved after treatment in the two groups （P 〈 0.01 ）, and there was no significant difference in blood pressure reduction between both groups. Conclusion Blood and urinary β2-microglobulin levels decrease and GFR level increases in different degree after treatment of levoamlodipine and irbesartan along with their effectively controling blood preasure, and the therapeutic effect of irbesartan is better than that of levoamlodipine. Irbesartan and levoamlodipine can protect the renal function in patients with essential hypertension.
