摘要
分析了４６例颅脑外伤所致精神障碍患者的临床资料。认为临床诊断时不应过分强调神经系统阳性体征和脑电图异常对本病的诊断价值，要对脑外伤史、临床特征、神经心理学等检查发现进行综合分析。在作出脑震荡后综合征诊断时，应注意排除导致这些症状的其他神经系统疾病。对颅脑外伤远期出现的发作性意识障碍应作出不同的亚型诊断。
Objective:To study diagnostic problems in patients with mental disorders caused by brain injury.Method:46 patients with mental disorders caused by brain injury were analyzed by reviewing their case records retrospectively.Results:20 patients had positive neurologicl signs.9 subjects had post-traumatic confusion during the non-acute period.80% of subjects developed mental disorders within 1 year after the brain injury.16 patients had other neurological diseases simultaneously.Of 18 patients EEGs examined,9 revealed abnormality.Discussion:In diagnosing mental disorders caused by brain injury,a comprehensive examination of injury history,clinical symptoms and neuro-psychological signs is important.Positive neurological signs and EEG results were insufficient for differential diagnosis.Other neurological diseases should also be excluded before a diagnosis of post-concussional syndrome is made.It is frequently difficult to differentiate schizophrenia from psychosis caused by long-interval brain injury. 〖HK
