摘要
目的:研究鹿茸多肽-PLGA复合膜对周围神经损伤修复的促进作用。方法:将Wistar大鼠一侧后肢坐骨神经切断,2个实验组(n=18)大鼠分别用15 mg.g-1和3 mg.g-12种浓度剂量型的鹿茸多肽-PLGA复合膜包裹神经缝合口,对照组(n=18)只做外膜缝合。分别在术后第2、4及6周,观察神经吻合口与周围组织黏连情况;测定小腿三头肌最大诱发电位;对吻合口周围的神经组织,分别行HE和Luxol fast blue染色;电镜观察神经纤维的再生情况。结果:在术后各观测时间点,实验组动物足底、足趾均无溃疡,神经吻合处与周围组织仅有轻度黏连,在术后6周时较2周时黏连明显减轻;术后2周时,坐骨神经支配的小腿三头肌诱发电位恢复率(%)测定结果,高剂量组为9.07±1.44,低剂量组为8.02±1.41,对照组为2.52±1.83,用药组恢复率明显高于对照组(P<0.01),高剂量组较低剂量组高(P<0.05);术后6周时,坐骨神经支配的小腿三头肌诱发电位恢复率(%)测定结果,高剂量组为49.87±9.69,低剂量组为50.11±6.11,对照组为30.31±6.32,用药组恢复率明显高于对照组(P<0.01),高剂量组较低剂量组高(P<0.05),而且术后6周时恢复率明显高于术后2周时(P<0.05);再生有髓纤维计数、直径、截面积恢复率(%),术后2周时,高剂量组19.56±5.87、35.35±3.23及20.68±3.21,低剂量组14.43±1.73、20.78±2.10及15.83±2.31,对照组9.56±l.97、8.32±10.65及4.64±1.36;术后6周时,高剂量组80.22±5.14、78.31±4.32及80.12±3.45,低剂量组72.66±5.31、73.45±2.56及64.06±5.10,对照组58.98±1.58、51.22±2.54及56.14±3 78,用药组恢复率明显高于对照组(P<0.01),高剂量组较低剂量组高(P<0.05),而且术后6周时恢复率明显高于2周时(P<0.05);电镜下用药组比对照组、高剂量用药组比低剂量用药组有髓纤维成熟明显,且术后6周时较术后2周更加明显。结论:鹿茸多肽-PLGA复合膜可提高大鼠周围神经损伤修复后的疗效。
Objective To study the rehabilitative effect of velvet antler polypeptides-PLGA compound membrane on peripheral nerve injury. Methods Two doses (3 and 15 mg · g^-1 ) of velvet antler polypeptides-PLGA compound membrane (thickness 50 μm) were used to wrap the sutures of severed sciatic nerves, which were compared with nerve sutures only. 2, 4 and 6 weeks post-operation, general morphological, electrophysiological, histological and electron microscopic observations and examinations were investigated. Results There were no ulcers in the nars and toes in the treatment group, light conglutination appeared in nerve anastomosis and around tissues. At 6th week post-operation, conglutination was alleviated obviously than 2 weeks post-operation; findings from latency inducing potentials of calf triceps dominated by sciatic nerves showed that recover ratios (%) in treatment group (2 weeks: 9.07±1.44, 8. 02±1.41; 6 weeks: 49. 87±9.69, 50.11±6.11) were better than those in control group (2 weeks:2. 52± 1.83; 6 weeks: 30.31 ±6.32) obviously in each time (P(O. 01); high dose group was better than low dose group (P〈0. 05), and 6 weeks post-operation recover ratios (%) were better than 2 weeks ones (P〈0. 05) ; regenerative myelination fiber counts, diameter and section area in treatment group were better than those in control group in each time obviously (P〈0. 01), high dose group was better than low dose group (P〈0.05), and recovery ratios at 6th week post-operation were better than 2 weeks ones (P〈0. 05) ; maturation of myelination fiber in high dose group was more evident than control and low dose groups respectively, 6 weeks post-operation maturation of myelination fiber was more obvious. Oonclusion Velvet antler polypeptides-PLGA compound membrane can elevate the curative effect of rehabilitation of injured peripheral nerves.
