摘要
目的观察剖宫产术后应用镇痛泵对产妇镇痛效果及术后对产妇泌乳情况的影响。方法将326例剖宫产产妇随机分为实验组与对照组各163例,实验组剖宫产术后保留硬膜外导管接镇痛泵给药,对照组术后即拔除硬膜外导管,按传统方法根据产妇的疼痛情况使用镇痛剂止痛。结果①两组术后镇痛效果比较,P<0．05,差异有统计学意义; ②实验组平均镇痛维持时间为68 h,对照组为3．5 h,两组比较,P<0．01,差异有统计学意义;③术后3 d产妇泌乳成功率实验组为96．9％,对照组为62．0％,两组比较,P<0．05,差异有统计学意义。结论镇痛泵能有效地解决剖宫产术后产妇切口疼痛的问题,减轻产妇对术后疼痛的恐惧感,促进产妇生理和心理的康复,促进乳汁的分泌,提高母乳喂养率。
Objective To discuss the analgesic effects and the impact of the application of analgesia pumps on lactation in post-cesarean women. Method 326 parturients undergoing cesarean sections were randomly divided into an experiment group, and a control group, with 163 cases in each group. The epidural catheters of the experiment group were carefully reserved, connected to Analgesia Pumps for medication infusion. The control group was allocated to accept traditional treatment. The epidural catheters of the subjects in the control group were removed immediately after operation. They were medicated with anodynes, the doses of which vary according to the clinical diagnosis of pain. Results Significant difference was found in the analgesic effects hetween the two groups （P 〈 0.05）. The analgesic effect maintained 68 hours in the experiment group and 3.5 hours in the control group and the result had statistics significance. Significant difference was found in post-cesarean lactation success ratio, with 96.9% in the experiment group and 62.0% in the control group （P 〈 0.05）. Conclusion Analgesia pump is very effective in relieving post-operative pain in the incisions of the mothers. It alleviates panic or algophobia, improves the secretion of milk, and raises the rate of breast-feeding. It also accelerates the recovery rate of mothers and infants.
