摘要
目的探讨Ⅳ型高脂血症患者血清高密度脂蛋白亚类含量及分布特征。方法采用双向电泳—免疫印迹检测法分析132例Ⅳ型高脂血症患者和163例正常对照者血清高密度脂蛋白亚类的组成及含量。结果与血脂正常组比较,Ⅳ型高脂血症患者血清前β1-高密度脂蛋白、高密度脂蛋白3a含量显著升高(P<0.01),而高密度脂蛋白2a、高密度脂蛋白2b含量显著降低(P<0.01)。无论是血脂正常者还是Ⅳ型高脂血症患者,女性前β1高密度脂蛋白、高密度脂蛋白3b含量均显著低于男性(P<0.05),而高密度脂蛋白2b则显著高于男性(P<0.01)。相关分析发现,Ⅳ型高脂血症患者血清甘油三酯、甘油三酯/高密度脂蛋白胆固醇比值与前β1-高密度脂蛋白、高密度脂蛋白3a含量显著正相关(P<0.01或P<0.05),与高密度脂蛋白2b、高密度脂蛋白2a含量显著负相关(P<0.01或P<0.05)。结论Ⅳ型高脂血症患者血清小颗粒高密度脂蛋白含量明显增加而大颗粒高密度脂蛋白含量明显减少,表明高密度脂蛋白成熟代谢过程受阻,胆固醇逆向转运作用减弱;此外,与男性相比,无论是血脂正常者还是Ⅳ型高脂血症患者,女性小颗粒高密度脂蛋白含量较少而大颗粒高密度脂蛋白含量较多。
Aim To study distributions and contents of high density lipoprotein (HDL) subspecies in type Ⅳ hyperlipidemic patients. Methods Apolipoprotein (apo)A Ⅰ contents of serum HDL subspecies in 132 type Ⅳ hyperlipidemic (Ⅳ- HIP) patients and 163 normal controls were determined by two-dimensional gel electrophoresis asaociated with immunodection method. Results Compared with normal controls, the contents of small-sized preβ1-HDL, HDL3a were significantly higher (P〈0.01), but those of large-sized HDL2a, HDL2b were significantly lower (P〈0.01). In addition, males hadsignificantly higher small-sized pre β1-HDL, HDL3b ( P 〈 0.05), but lower large-sized HDL2b ( P 〈 0.01 ) than females in both normal controls and Ⅳ-HLP patients. Correlation between HDL subspecies and lipids in Ⅳ-HIP patients showed that triglyceride (TG) and TG/HDLC ratio were positively correlated with pre β1 -HDL, HDL3a ( P 〈 0.01 or P 〈 0.05 ), but negatively correlated with HDL2b, HDL2a(P〈0.01 or P〈0.05). Conclusions The partide size of HDL shifted towards smaller sizes, which, in turn, indicates that reverse cholesterol transport might be weakened and HDL maturation might be abnormal. Moreover, males probably have higher pre β1-HDL, HDL3b, but lower HDL3b than females in general population.
