多器官功能障碍综合征患者外周血单个核细胞凋亡的研究 被引量:6

摘要 目的观察多器官功能障碍综合征(M ODS)患者外周血单个核细胞(PBM C s)凋亡及其B cl 2和p53的mRNA表达。方法M ODS患者25例为M ODS组,18名健康志愿者为正常对照组。采用流式细胞仪、电子显微镜、荧光显微镜、DNA琼脂糖凝胶电泳法,观察患者确诊48 h内及正常对照组的PBM C s凋亡;采用逆转录聚合酶链反应(RT PCR)检测PBM C s B cl 2和p53的mRNA表达。结果M ODS患者PBM C s出现典型的细胞凋亡形态学改变,其凋亡率明显高于对照组〔(25.4±9.2)%比(15.9±6.8)%,P<0.01〕;M ODS组的PBM C s凋亡细胞数明显高于正常对照组〔(1.040±0.096)个/高倍视野比(0.235±0.028)个/高倍视野,P<0.05〕。M ODS组PBM C s的B cl 2 mRNA表达量明显低于正常对照组(0.11±0.09比0.19±0.06,P<0.05);p53 mRNA表达量明显高于正常对照组(0.45±0.09比0.25±0.12,P<0.05)。结论M ODS患者PBM C s凋亡异常增高,B cl 2 mRNA表达下调,p53 mRNA表达上调。提示M ODS患者存在细胞凋亡异常,凋亡相关基因表达异常。 Objective To study apoptosis of peripheral blood mononuclear cells (PBMCs) in patients with multiple organ dysfunction syndrome (MODS) and its associated gene Bcl - 2 and p53 expression. Methods Twenty -five patients with MODS and 18 healthy volunteers were enrolled for the study. Flow cytometry assay, electron microscopy, acridine orange -ethidium bromide staining and fluorescence microscopy, DNA agarose gel eleetrophoresis were used to identify and quantify apoptosis of PBMCs, Reverse transcription - polymerase chain reaction (RT - PCR5 was used to quantify Bcl - 2 mRNA and p53 mRNA expression, Results Typical morphological features of apoptotic PBMCs were observed in the patients, The apoptosis rate in M(SDS group was (25.4±9.2)%, and it was higher than that of control group (15.9±6. 850//00 (P%0, 01). The number of apoptotic cells was (1. 040±0. 096)/high power field in MODS group, and it was higher than that in control group (0. 235±0. 0285/high power field (P〈0. 05). Bcl- 2 mRNA expression of PBMCs in patients was significantly lower than that of healthy volunteers (0, 11±0. 09 vs. 0. 19±0, 06, P〈0.05), while p53 mRNA expression was higher in patients than thai of healthy volunteers (0.45±0.09 vs, 0. 25±0, 12, P〈0. 055. Conclusion PBMCs apoptosis in patients with MODS is increased abnormally. The expression of Bcl - 2 mRNA in patients is decreased while p53 mRNA expression is increased. The results suggest that abnormal apoptosis of monocytes as well as abnormal expression of apoptosis associated genes occur in patients with MODS.
