缓解期慢性阻塞性肺病患者的营养干预 被引量:7

摘要 目的:探讨缓解期慢性阻塞性肺病营养干预的原则、目标、涉及的潜在影响因素及可行的应对措施在慢性阻塞性肺病营养支持范畴中的意义,以提高营养不良的慢性阻塞性肺病患者疾病康复和生活质量。资料来源:应用计算机检索Medline2000-01/2004-12关于慢性阻塞性肺病营养支持研究方面的文章,检索词“chronicobstructivepulmonarydisease(COPD),nutritionintervention,nutritionsupport,dietrecommendation”,并限定文章的语言种类为English。同时利用万方数据库、中国医院数字图书馆、中国期刊网2001-01/2005-11的相关文章及出版物,限定语言种类为中文,检索词“慢性阻塞性肺病、营养支持”。资料选择:对资料进行初审,纳入标准:①关于缓解期慢性阻塞性肺病营养干预的原则、目标、影响因素及措施。②对具体临床实验研究的回顾总结。排除标准:排除重复性的实验研究。资料提炼:共收集到符合上述要求的文献75篇,排除53篇重复性研究,对符合标准的23篇文献进行综述,其中12篇关于慢性阻塞性肺病营养干预的实验研究,1篇营养支持的随机对照试验Meta分析,10篇讲座和治疗指南类综述。资料综合:营养干预对改善慢性阻塞性肺病能量负平衡、免疫功能下降和心理状态不适有重要意义。营养干预细分近期目标可强化治疗手段的具体实施。慢性阻塞性肺病存在进食时呼吸节律的改变、胃肠道功能紊乱、药物和精神状态影响和合并其他慢性疾病等多项可能影响营养干预的潜在因素。食物组成比例的个体化调整、种类的优化选择、良好饮食习惯的养成和定期监测和管理的具体措施有助于取得更确切的治疗效果。结论:对营养干预在慢性阻塞性肺病长期治疗方面有了进一步的深刻认识,明确了及时的营养干预和合理的饮食指导对慢性阻塞性肺病患者的疾病康复及生活质量的提高都有显著意义,是改善疾病预后的有效手段。 OBJECTIVE: To investigate the significance of principles, purposes, related potential factors and feasible measurements of nutrien intervention in chronic obstructive pulmonary disease (COPD) in the remission phase, so as to accelerate the rehabilitaion and improve the life qualities of patients with malnutrition COPD. DATA SOURCES:A computer-based search of the Medline database for relevant articles about COPD nutritional support published in English between January 2000 and December 2004 was performed by using the keywords of "chronic obstructive pulmonary disease (COPD), nutrition intervention, nutrition support, diet recommendation". Meanwhile, Wanfang database, China Hospital Digital Library (CHDL) and China Journal Net were scanned with computer for related articles published in Chinese from January 2001 to November 2005, and the keywords were "chronic obstructive pulmonary disease, nutrition support". STUDY SELECTION:The data were selected primarily. Inclusive criteria: ①Related principles, purposes, measurements and influencing factors of nutrien intervention in COPD in the remission phase. ②Retrospective summary about specific clinical experimental researches. Exclusive criteria: Repetitive experimental studies. DATA EXTRACTION:A total of 75 literatures were in accordance with the criteria, 53 repetitive articles were excluded and 23 included literatures were summarized including 12 experimental researches about COPD nutrien intervention, one meta-analysis of randomized control trials (RCT) and 10 lectures and curative guidance reviews. DATA SYNTHESIS:Nutrien intervention plays a significant role in ameliorating energy negative balance, immunological functional decreasing and mental upset of COPD. The specific measurements of therapeutic means could be enhanced by subdividing purposes of long-term and immediate goals. Potential influencing factors such as changes of respiratory rhythm, functional disturbances of gastrointestinal tract, side effects of medicine and mental status complicated with other chronic diseases existed in nutrien intervention. Formation, regular monitoring and specific managements on individual adjustment of food ratio, optimization of class and good eating habit were good to the curative effects. CONCLUSION: Nutrien intervention in the treatment of COPD is further acknowledged. In time nutrien intervention and reasonable diet restructions are important to the rehabilitation and the improvement of life quality in patients with COPD, it is an effective means of ameliorating disease prognosis.
