瘦素、瘦素受体和血管内皮生长因子在胃癌中的表达及其在浸润和转移中的作用 被引量:4

Expression of leptin, leptin receptor and vascular endothelial growth factor in gastric carcinoma and their roles in invasion and metastasis

摘要 目的探讨瘦素、瘦素受体和血管内皮生长因子(VEGF)在胃癌发展、浸润和转移中的作用及瘦素和VEGF的相互关系。方法应用免疫组织化学SP法检测20例正常胃黏膜、40例胃癌前病变和60例胃癌组织中瘦素、瘦素受体和VEGF表达情况。结果瘦素在进展期胃癌组织中的表达明显增强,其阳性率(77.8%)显著高于正常胃黏膜、慢性萎缩性胃炎伴肠化、不典型增生及早期胃癌组织(20.0%、35.0%、45.0%、46.7%,均P<0.05);瘦素表达与组织学分级、浆膜浸润和淋巴结转移有关(P<0.05)。瘦素受体阳性表达率在胃癌发展过程中无统计学意义(P>0.05)。进展期胃癌组织中VEGF阳性表达率75.6%显著高于正常胃黏膜、慢性萎缩性胃炎伴肠化、不典型增生、早期胃癌组(15.0%、35.0%、45.0%、46.7%,均P<0.05);VEGF表达与组织学分级无关(P>0.05),与浆膜浸润、淋巴结转移有关(P<0.05);瘦素与VEGF在胃癌组织中的表达存在显著正相关关系(rs=0.673,P<0.01)。结论瘦素、瘦素受体和VEGF均参与胃癌的发展、浸润和转移。 Objective To evaluate the roles of leptin, leptin receptor and vascular endothelial growth factor (VEGF) in the development,invasion and metastasis of gastric carcinoma(GC) as well as their interaction. Methods Sreptacidin peroxidase(SP) technique was adopted to examine the expression of leptin,leptin receptor and VEGF in 20 eases of normal gastric mucosa(NGM) ,40 cases of precancerous lesions and 60 cases of GC. Results There was a significantly much higher positive rate(77.8%)of leptin protein in advanced gastric carcinoma than that in NGM, chronic atrophic gastritis with intestinal metaplasia (CAG+IM) ,atypical hyperplasia(Dys) and early gastric carcinoma tissues(20.0%,35.0%, 45.0%, 46.7% ; P 〈 0.05, respectively). Leptin positive expression was related with the histologic stage,serosa infiltration and lymph node metastasis ( P〈0. 05). With the development of GC, the positive rate of leptin receptor varied unobviously ( P 〉0.05). The positive rate of VEGF expression was 75.6% in advanced gastric carclnoma,which was much higher than that in NGM, CAG+IM, Dys and early gastric carcinoma (15.0%, 35.0% ,45.0%,46.7% ; P〈0.05 ,respectively). There was no relationship between VEGF expression in GC and the histologic stage (P〉0.05). VEGF positive expression was related with the serosa infiltration and lymph node metastasis ( P〈0.05). By statistical analysis, the expression of leptin in GC was significantly correlated with that of VEGF( r5 =0. 673, P 〈0.01). Conclusion Leptin,leptin receptor and VEGF contribute to the development,invasion and metastasis of GC.
