摘要
对35例不明原因晕厥病人及15例正常人行直立倾斜试验(HUT)检查,观察血压、心率及血浆去甲肾上腺素(NE)浓度的变化。结果示平卧位阳性(Ⅰ组)、阴性(Ⅱ组)及对照(Ⅲ组)病例无显著差异(P>0.05)。倾斜后5分钟,3组心率、舒张压、NE均较平卧位有所上升(P<0.05),但3组间无显著性差异(P>0.05)。HUT结束时,心率、舒张压Ⅰ组表现为较前明显下降(P<0.05),而Ⅱ、Ⅲ组较前又有所上升(P<0.05),Ⅰ组与Ⅱ、Ⅲ组间差异非常显著(P<0.01)。血中NE浓度3组均较前又有所增加(P<0.05),3组间差异无显著性(P>0.05)。说明HUT中,不能仅依据血中NE水平来判断体内交感神经张力的高低。
We performed the head up tilt test(HUT) on 35 patients with syncope of unknown origin and 15 healthy persons, observing the changes of heart rate(HR), blood Pressure(BP) and level of plasma norepinephrine(NE). The result showed that there were no difference in HR, BP and NE( P >0.05) among the positive (group Ⅰ), the negative (group Ⅱ) and the controls (group Ⅲ) during supine position. All three groups increased more in HR, DBP and NE in the tilt position 5 minutes after tilting than in the supine position before tilting( P <0.05), but there were no difference among three groups( P >0.05). At the end of the HUT, group Ⅰ had obvious decrease of HR and BP( P <0.05) while groups Ⅱ and Ⅲ had more increase of them than ever ( P <0.05), there were significant difference between group Ⅰ and group Ⅱ,Ⅲ( P <0.01). All three groups had more increase of the level of plasma NE than ever ( P <0.05), but there were still no significant difference among them. It showed we could not justify the sympatheitc tone only by the level of the plasma NE during HUT.
