摘要
对服用雷公藤糖浆患者的69份精液标本和2例睾丸活检组织进行了光镜和透射电镜形态学观察。结果显示:服药15日以上者,其精液中可见大量脱落的精子细胞和精母细胞,这些细胞均有明显的超微病理变化。皋丸曲细精管中精子细胞和精母细胞脱落至净,支持细胞的紧密连接有超微病理变化,但是,精原细胞未见形态学异常。提示雷公藤的“毒性”物质可能主要作用于睾丸曲细精管连腔小室内的生殖细胞。该结果为服用雷公藤可影响精子发生,停用雷公藤后精液常规能恢复正常提供了形态学依据。
To investigate the mechanism of action of Tripterygium Wilfordii Hook f. (TWH) on human spermatogenesis and its target cells.Patients and Methods Sixty-nine semen specimens from 52 volunteers with ankylosing spondylitis treated with TWH and testicular biopsies from 2 of them were investigated with light and electron microscopy.Results A large number of exfoliated spermatids and spermatocytes were present in 19 semen specimens obtained after 15 or more days of TWH treatment. Marked pathological alterations were found at ultrastructural level in these exfoliated cells. In the two cases of testicular biopsies, there was exfoliation of sper-matids and spermatocytes. Moreover, ultrastructural changes were demonstrated in the tight junctions between the Sertoli cells. However, no morphological abnormalities were seen in the spermatogonia.Conclusions The toxic components in TWH might act only on those germinal cells inside the adluminal compartment of the seminiferous tubules. This study provides a morphological support to the fact that TWH can definitely interfere human spermatogenesis, but the effect is reversible after cessation of TWH.
