摘要
【目的】通过新生儿尿N-乙酰-β-D-氨基葡萄糖苷酶(Nacetyl—beta-D-Glucosaminidase,NAG)的筛查对新生儿病区患儿的肾小管功能进行总体的早期评价。【方法】对2003年1月~2004年7月共1067例住院新生儿按不同疾病分多组行尿NAG、尿常规检测及147例血尿素氮检查。【结果】住院新生儿尿NAG异常率极显著高于尿常规及血尿素氮异常率。其中新生儿轻度窒息、早产儿、新生儿肺炎和高胆红素血症患儿尿NAG活性异常升高发生率显著高于尿常规;轻度窒息和早产儿血尿索氮异常率显著低于尿NAG异常率。而重度窒息、极低出生体重儿、新生儿破伤风、肺透明膜病、吸入综合征患儿尿NAG活性异常率同尿常规和血尿素氮异常率比较差异无显著性。【结论】住院新生儿不同程度存在肾小管功能损害,新生儿尿NAG较尿常规和血尿素氮反应肾小管功能变化更灵敏,是评价新生儿早期肾小管功能异常的灵敏指标。
[Objective] To study early prediction renal tubular damage in newborn infants in hospital by urinary N- acetyl-beta-D-Glucosaminidase (NAG)activity. [Methods] Urinary NAG- activity and routine were measured in 1 067 newborn infants ,blood urea nitrogen(BUN) were measured in 147 newborn infants in hospital from January 2003 to July 2004 . They were divided into some groups according to different diseases. [Results] There were significant differences in abnormal rates hetween urinary NAG activity and routine in all of infants, the same result to urinary NAG activity and BUN. There were higher abnormal rates of uric NAC- activity than urinary routine in mild asphyxia and prematurity and pneumonia and hyperbilirubinemia ; There were not significant difference between abnormal rates of uric NAG than urinary routine or BUN in newborn infants with serious asphyxia and very low body weight and neonatal respiratory syndrome,and aspiration syndrome. [Conclusion] There are different degree renal, tubular damage in newborn infants in hospital . The changes of uric NAG activity can evaluate renal tubular injury early. It was more sensibility and facility to predict renal tubular function according uric NAG than urinary routine and BUN in newborn infants with mild asphyxia and prematurity. Docter can find renal function failure early and treat it early. The renal function failure can be prevented.
