摘要 目的研究胶质纤维酸性蛋白在涎腺肿瘤中分布规律,并与波形丝蛋白、细胞角蛋白和S-100蛋白的分布规律进行比较,以探讨涎腺肿瘤组织发生学。方法免疫组化ABC法。结果在正常涎腺中,肌上皮细胞呈胶质纤维酸性蛋白阴性反应。在紧邻肿瘤或已受癌浸润的腺体中,围绕着闰管和萎缩的腺泡外围有层扁平的细胞,它们呈胶质纤维酸性蛋白阳性。在多形性腺瘤中,导管双层结构的外层瘤细胞、肿瘤性肌上皮细胞、粘液样细胞和软骨样细胞均呈胶质纤维酸性蛋白阳性反应。胶质纤维酸性蛋白还见于腺样囊性癌和乳头状囊腺癌癌巢的基底部。这些细胞同时表达波形丝蛋白、细胞角蛋白和S-100蛋白。结论涎腺肿瘤可能起源于涎腺闰管的基底细胞,该基底细胞可能与神经外胚层关系密切。 In order to study the histogenesis of salivary gland tumors,the immunohistochemical expression of glial fibrillary acidic protein(GFAP)in salivary gland tumors was studied and the results were compared with those for vimentin,cytokeratin(K8.12)and S-100 protein.Methods Immunohistochemical ABC methods.Results In normal salivary gland far from tumors,the myoepithelial cells did not express GFAP,In glandular tissues adjacent to tumor or invaded by carcinoma,GFAP was detected in flattened and spindle shaped cells that surrounded intercalary ducts and atrophic acinar structures.In pleomorphic adenoma,the non-luminal cells in tubulo-ductal structures and modified or neoplastic myoepithelial cells which formed solid,myxoid and chondroid structures showed intense immunoreactivity for GFAP.GFAP was also detected in basally located tumor cells in adenoid cystic carcinoma and papillary cystadenocarcinomas.Those cells correspoding to the GFAP positive cells were also positive for vimentin,cytokeratin(K8.12)and S-100 protein.Conclusion It is suggested from the present study that the salivary gland tumors may originate from basal cells of intercalary ducts and this cell may closely relate with neural ectoderm.
