健康教育对重型肝炎患者选择人工肝治疗决策影响的研究 被引量:19

The Effect of Health Education on the Decision to Accept Artificial Liver Therapy

摘要 目的通过对重型肝炎患者及其家属进行有计划、有目的的健康教育,从而影响患者对人工肝治疗选择的决策。方法研究应用类试验的研究方法,通过抽样,将2001年12月~2005年4月在西安交通大学医学院第一附属医院传染科住院的245例重型肝炎患者分为健康教育组和非健康教育组。非健康教育组只给予传染科常规教育,健康教育组除了传染科常规教育外,人工肝治疗室护士应用护理程序评估患者及其家属的价值观、健康信念、人工肝有关知识。然后制定有针对性的教育计划,进行有关人工肝的健康教育。统计两组选择人工肝治疗的例数、治疗时机及一般资料,采用SPSS 11.5软件进行X^2检验。结果非健康教育组选择人工肝治疗的患者经济状况明显好于健康教育组(P〈0.05);健康教育组选择人工肝治疗的患者显著多于非健康教育组,而且选择时机多在疾病的早、中期(P〈0.05);健康教育组的治疗效果优于非健康教育组(P〈0.05)。结论健康教育可以影响重型肝炎患者对人工肝治疗选择的决策,使其能够在疾病的早、中期选择人工肝治疗,提高治疗效果,降低病死率,为患者挽救生命赢得时间。 Objective Through educationg the patients suffering from severe hepatitis and families systematically and objectively to effects their decision on the treatment choice of artificial liver. Methods Quasi-experimental study. Through the accidental sampling, the 245 of severe hepatitis patients recorded from December, 2001 to April, 2005 in infection section, the First Hospital of Medical College in Xi'an Jiaotong University, are divided into two groups: health education group & non health education group.. The non health education only offers the normal medical care education in infection section. However, in the health education group, it offers not only the normal medical care education in infection section, but also supplies the methodology of artificial liver therapy implemented by the nurses in the therapy room, which evaluates the patients and their families'value, health belief and concerned acknowledge and knowledge of artificial liver. Then scientifically make the education plans and give the health education of the artificial liver. The two groups of cases of accepting artificial liver therapy, therapy occasions and necessary normal information about it are collected and the outcomes are statically analyzed with X^2-test, Microsoft ware SPSS 11.5. Results The financial situation of the patients who have accepted the artificial liver therapy in non health education group is much better than that of health education groups (P 〈 0.05); The number of patients who have accepted the artificial liver therapy in health education group is much larger than that of non health education group's. And also, the occasion chosen usually is in the early or middle period (P 〈 0.05); The remedy result in health education group is much better than that of non health education's (P 〈 0.05). Conclusion Health education has influences in making the decision and encouraging the patients to choose and accept artificial liver therapy as soon as possible in order to improve the efficiency of the severe hepatitis treatment and decrease the mortality of the severe hepatitis.
