摘要
目的探讨经腹根治胃上部癌的手术适应症及对本术式的评价。方法回顾总结1994-2004利用管型消化道吻合器经腹根治手术的125例胃上部癌的临床资料。结果经腹手术创伤小,出血少,并发症少,安全性较好,但亦有局限性。结论经腹手术路径的选择主要应根据肿瘤的具体情况而定,合理掌握指征,对于非贲门上部癌、食管侵犯≤2cm者以及BormannⅠ、Ⅱ型的胃上部癌应优先考虑经腹手术。
ObJective To explore the indications for the radical resection of upper gastric carcinomas in abdominal cavity and evaluation for the pathway of which. Methods 125 eases with superior advanced gastric carcinoma received radical operation between 1994 and 2004 were reviewed. Of these, all eases had abdominal cavity operation by radical resection of proximal portion of the stomach or total gastrectomy with tube shaped digestive tract stapling instrument. Results Comparative analysis indicated that abdominal had an advantage in reducing operative injury, decreasing hemorrhage and complication, Conclusion Selection of operative pathway must depend upon tumor conditions. Although abdominal resection is a securer mode, we should be paid attention the indications. Such eases with upper gastric cancer as non - cardiac part, esophageal involvement ≥ 2cm or Borrmann Ⅰ , Ⅱ type, abdcninal resection ought to be toppriority.
