肿瘤耐药相关标志在肝细胞癌中的表达及其意义 被引量:7

摘要 目的探讨P-糖蛋白(Pgp)、多药耐药相关蛋白(MRP)和谷胱苷肽S-转移酶-π(GST-π)在肝细胞癌(HCC)中的表达及其临床意义。方法采用免疫组化技术对HCC和癌旁组织中Pgp、MRP和GST-π的表达进行检测。结果HCC和癌旁组织中Pgp、MRP和GST-π的阳性率分别为59.1%、54.5%、65.9%和22.7%、20.5%、34.1%。HCC中Pgp、MRP和GST-π阳性率明显高于癌旁组织(χ12=12.2,χ22=10.6,χ32=11.7,P<0.01)。分化不良的HCC中的Pgp阳性率(80%)明显高于分化较好(41.6%)的HCC(χ2=6.7,P<0.01)。MRP和GST-π在分化不良(65%、80%)和分化较好(45.8%、54.2%)的HCC中的阳性率相比差异无统计学意义(χ12=2.4,χ22=3.0,P>0.05)。结论Pgp、MRP和GST-π在HCC中呈高表达。Pgp与HCC分化有关,MRP和GST-π与分化无关。联合检测耐药相关标志可以协助临床判断预后和疗效。 Objective To invetstigate the expression and significance of P-glycoprotein (Pgp), mulitidrug resistance-associated protein (MRP) and glutathion S-transferasex(GST-π) in hepatocellular carcinoma (HCC). Methods Pgp, MRP and GST-π were determined in HCC and tumor-surrounding tissues by immunohistochemistry. Resultes The positive rates of Pgp, MRP and GST-π in HCC were 59.1%, 54.5% and 65.9%, and in tumor-surrounging tissues were 22.7%, 20. 5% and 34.1% respectively. The positive rates of Pgp, MRP and GST-π in HCC were higher than that in tumor-surrounding tissues (x1^2 = 12.2, x2^2 = 10. 6,x3^2 = 11.7, P〈0.01). The positive rates of Pgp in poorly-differenciated HCC were higher than that in well-differenciated (80% vs 41.6%. X1^2= 6.7, P〈0.01). There was not a significant correlation between the positive rates of MRP and GST-π in pooly-differenciated HCC(80%) and that in well-differenciated (65% vs 45.8%, 80% vs 54.2%. x1^2 =3.5,x2^2=3.0, P〉0.05). Conclusion Expression of Pgp, MRP and GST-π are high in HCC. Expression of Pgp is closely correlated to differenciation of HCC, but MRP and GST-π are not. Combined determination of drug-resistance-associated markers may help judge prognosis and effect after chemotherapy.
