摘要
目的探讨血清白介素12、白介素4及γ-干扰素在过敏性紫癜患儿中的表达及其临床意义。方法本研究采用ELISA方法检测了35例过敏性紫癜患儿(其中13例合并肾脏损害,并随访8例患儿急性期和缓解期血清细胞因子变化)以及20例正常健康儿童的血清白介素12、白介素4及γ-干扰素水平,分别比较急性期和缓解期以及有和无合并肾损害的过敏性紫癜患儿的细胞因子水平;分析白介素12水平与白介素4及γ-干扰素是否存在相关关系。结果1.HSP患儿的血清IL-12及IFN-γ水平低于正常对照组,而IL-4水平高于正常对照组(p值<0.01);2.无肾损害HSP组与HSPN组的血清IL-12、IL-4、IFN-γ水平均无统计学差异;3.HSP患儿急性期的血清IL-12和IFN-γ低于缓解期,p值分别为0.041和0.038;而IL-4高于缓解期,p值<0.01。4.HSP患儿的血清IL-12水平与IFN-γ存在中等程序正相关关系(r=0.437,p<0.01)。结论血清IL-12、IL-4和IFN-γ可能参与HSP的发病过程,但HSPN的发生与其异常并无相关性,造成HSP肾损害的原因尚需进一步探讨。
Objective To investigate the changes of serum levels of IL - 12 ,IL-4 and IFN-γ, and probe their clinical significance in children with HSP/HSPN. Methods To examine the serum levels of IL-12 ,IL-4 and IFN-γof children with HSP/HSPN by ELISA, and analyze the changes and the correlation between the aforementioned cytokines in children with HSP/HSPN. Results 1. serum levels of IFN- γand IL-12 of the patients with HSP were lower than those of the control group （P 〈0.01 ） ;A serum level of IL-4 of patients with HSP was higher than those of the normal control group P 〈0.01 ） ; 2. No significant difference was found between non-renal involvement HSP and HSPN groups on serum levels of IL- 2 ,IL-4 and IFN-γ; 3. The serum levels of IL-12 and IFN-γat disease onset were lower than those in remission （the value of p was 0. 041 and 0. 038 respectively）, and the serum levels of IL-4 at disease onset was higher than those in remission （ the value of p was 〈0. 01 ） ; 4. The serum IL-12 levels were positively correlated to the serum IFN-γ levels of patients with HSP（ r =0. 437 ,p 〈0. 01 ）. Conclusions The results imply that serum IL-12 ,IL-4 and IFN-γmay play role in the pathogeny of HSP,hut the cause resulting in the renal involvement of HSP is still needed more researches.
