摘要
目的:观察调脂降糖片联合二甲双胍片治疗肥胖类2型糖尿病的临床疗效。方法:将60例患者随机分为2组各30 例。2组均给予基础治疗和二甲双胍片治疗,治疗组配合调脂降糖片(组成:水蛭、大黄、黄连等)治疗。观察2组治疗前后中医症候及体重(WT)、体重指数(BMI)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素敏感指数(ISI)、组织性纤溶酶原激活物(t- PA)、纤溶酶原激活物抑制物(PAI)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白-胆固醇(LDL-C)、高密度脂蛋白-胆固醇 (HDL-C)等指标的变化。结果:治疗后2组WT及BMI有明显下降,与治疗前比较,差异均有非常显著性意义(P<0．01),2组均差值比较,差异均有显著性意义(P<0．05)。治疗后2组FBG、FINS及ISI均有改善,与治疗前比较,差异有显著性或非常显著性意义(P<0．05,P<0．01);治疗后2组FBG比较,差异有显著性意义(P<0．05)。治疗组治疗后TG、TC、LDL-C、 HDL-C均得以改善,其中TC降低最为明显,与对照组治疗后比较,差异有显著性意义(P<0．05)。治疗后治疗组PAI、对照组t-PA均有所改善,与治疗前比较,差异有显著性意义(P<0．05)。结论:调脂降糖片联合二甲双胍片治疗肥胖类2型糖尿病患者能改善临床症状,减轻体重,降低体重指数,改善糖、脂代谢紊乱,并能显著增加胰岛素敏感性,其疗效明显优于单用西药。
Objective： To observe the curative effect of Tiaozhi Jiangtang Pian （TJP） associated with metformin tablet（MT） for obese type Ⅱ diabetes mellitus （ODM）. Methods; 60 cases were allocated to 2 groups randomly （each group 30 cases）, and basic treatment and MT were given in both groups, the treatment group added with TJP （composed of Hirudo, Radix et Rhizoma Rhei, Rhizoma Coptidis, etc. ）. The TCM syndrome pattern before and after treatment, body weight （BW）, body mass index （BMI）, fasting blood glucose （FBG）, fasting insulin （FINS）, insulin sensitivity index （ISI）, tissue plasminogen activator （t- PA）, plasminogen activator inhibitor（PAl）, triglyceride （TG）, total cholesterol （TC）, LDL- C and HDL- C were observed in both groups. Results; After treatment, the BW and BMI of both groups were markedly decreased as compared with before treatment （P 〈 0.01 ）, the difference before and after treatment in both groups being significant （P 〈 0. 05）; FBG, FINS and ISI in both groups were improved, the difference being significant or very significant as compared with before treatment （ P 〈 0. 05, P 〈 0.01 ） ; FBG in both groups differed significantly （ P 〈 0. 05） ; TG, TC, LDL - C and HDL- C in treatment group were improved, especially TG as compared with after treatment （ P 〈 0. 05）; PAI, t - PA were improved in both groups as compared with before treatment （P 〈 0.05）. Conclusion： TJP associated with MT can relief the clinical symptoms, decrease BW and BMI, improve glucose and lipid metabolism, indicating that TJP associated with MT can markedly increase ISI and its effect is superior to western medicine alone.
