摘要 目的探讨肥胖儿童青少年血脂异常、瘦素抵抗与胰岛素抵抗的关系。方法检测201例肥胖无糖代谢异常儿童,40例肥胖伴糖代谢异常儿童和230例正常体重儿童的空腹血糖、空腹血胰岛素、血脂、血瘦素水平,以胰岛素抵抗指数评价胰岛素抵抗。结果肥胖无糖代谢异常组空腹血糖、瘦素显著高于对照组;血甘油三脂、载脂蛋白B、低密度脂蛋白-胆固醇水平以及空腹血胰岛素水平、胰岛素抵抗指数在对照组、肥胖无糖代谢异常组、肥胖伴糖代谢异常组三组间均呈显著上升趋势;甘油三脂、瘦素与胰岛素抵抗指数呈显著正相关。结论肥胖儿童青少年具有明显的脂代谢紊乱、高瘦素血症和瘦素抵抗状态,并与胰岛素抵抗密切相关。 Objective The aim of present study was to investigate the relationship between dyslipidaemia,leptin resistance and insulin resistance through analyzing the metabolic changes in children and adolescents with obesity. Methods Subjects were divided into three groups: group one(children and adolescents with obesity without impaired glucose regulation,201 cases) ,group two( children and adolescents with impaired glucose regulation, 40 cases), group three( normal My weight group, control group, 230 cases ). Metablic changes ( FPG, TG,Tch, ApoA, ApoB, HDL- C, LDL- C, FINS, leptin ) were evaluated. Results The FPG level and leptin level of group one is statistically higher than that of control group. Significant increasing tendency of TG, ApoB, LDL-C, FINS level and HOMA-IR value was found in the three groups( group two 〉 group one 2〉 group three) ;There was a positive correlation between HOMA-IR and two other parameters (TG and leptin). Conclusions Obvious dyslipidaemia and leptin resistance is found in obese children and adolescents. The two factors may play a role in the development of insulin resistance.
