摘要
为给MR图像精确定位肝内占位性疾变提供解剖学依据,利用20例腹部连续横断层标本、41例胎肝管道铸型和30例活体正常肝MR横断图像,研究了肝段在横断面上的划分,正中裂为下腔静脉左前壁至肝中静脉长轴或胆囊窝中点的连线;左叶间裂的上部为左叶间静脉至下腔静脉左前壁的连线,门静脉左支矢状部和肝圆韧带裂是其中、下部的天然标志;肝左静脉行于左段间裂内;右叶间裂为肝右静脉长轴至下腔静脉右壁的违线;门静脉左支用以区分段IVa与IVb,而门静脉右支是右以间裂的标志;背裂为下腔静脉右壁至静脉韧带裂右端或门静脉的孤形线.文内还探讨了易致错分肝段的一些因素.
Cross sectional division of Couinaud's hepatic segments were investigated in 20 sets of horizontal serial sections of adult cadavers,41 fetal liver corrosion casts and 30 MR imaging. The medial fissure is the line from the antero-left wall of inferior vena cava (IVC) to the long axis of middle hepatic vein or to the midpoint of gallbladder fossa. The superior section of left interlobar fissure is the line from the left interlobar vein to the antero-left wall of IVC, and the left portal vein and the fissure of round ligament provide natural landmarks for the middle and inferior sections of the left interlobar fissure. The left hepatic vein runs in the left intersegmental fissure. The right interlobar fissure is the line from the right wall of IVC to the long axis of right hepatic vein. The right portal vein acts as the landmark of the right intersegmental fissure. The dorsal fissure is a curve from the right wall of IVC to the right end of venous ligament fissure or to the portal vein.
