摘要
呼吸指数(RI)是肺泡-动脉氧压差与动脉氧分压之比。其值受心输出量、混合静脉血氧分压、肺内分流、通气/血流比、通气状况及弥散功能的影响,是反映呼吸功能的重要指标。通过对80例低氧血症危重病患者的RI分析显示,RI的改变可确定低氧血症的存在与程度的变化,动态观察RI可评价低氧血症的疗效和预后。建议在ICU病房将RI与血气指标配合应用,以便更好地反映病人体内病理生理变化。
The respiratory index(RI)is definedas the ratio of alveolar- arterial oxygen gr adient(A-aDO2 diff)and arterial oxygen tension(PaO2)ie,RI=Its value is influenced by such factors determining the exchange of oxygen between inspiredgas and blood as cardiac output,mixed venous oxy-gen tension ,shunting of unsaturated blood across thepulmonary circulation,i nequalities of pulmonary ven-tilation,ventilation/perfusion mismatching in indi-vidual alveoli and diffusion limitation across the alve-olar walls,Our observation in eighty patients withhypoxemia showed that RI can provide an acurate es-timate of the existence and degree of hypoxemia aswell as being an important index of respiratorv func-tion.DvnamiC observation also can evaluate boththerapeutic effect and prognosis of hypoxic patients;Its use combined with blobd gas analysis in order toreflect pathophysiologic changes in patients with hy-poxemia in critical Care unit is recomrnended。
