复杂髋臼骨折的手术入路与内固定方法的选择 被引量:17

摘要 目的探讨复杂髋臼骨折手术的理想入路和内固定方法,以提高疗效.方法采用改良Smith-Petersen切口手术治疗15例髋臼复杂型骨折,并按骨折的不同类型分别用钢丝张力带、普通螺钉、可吸收螺钉、AO拉力螺钉、重建钢板等固定.结果随访5~12个月,平均7.6个月,13例达解剖复位,术后4例出现并发症.功能参照美国矫形外科研究院标准评价,优8例,良5例,可2例.结论改良Smith-Petersen入路具有显露范围大、肌肉剥离少、异位骨化率低等特点,是复杂髋臼骨折手术的较好入路;重建钢板可作为复杂髋臼骨折固定的首选材料. Objective To investigate the ideal surgical approach and internal fixation for complex acetabular fracture to improve the treatment effect. Methods 15 cases with complex acetabular fractures were treated through improved Smith-Petersen approach with tension band, compression screw, absorbable screw, AO lag screw and reconstruction plate fixation according to the fracture type, respectively. Results All cases were followed up for 5 - 12 months,7.6 on average. 13 cases got anatomic reduction. 4 got complications. The function recovery was evaluated by AAOS standard, which was excellent in 8 cases, good in 5 and fair in 2. Conclusions The improved Smith-Petersen approach ean provide sufficient exposure with less muscular strip and less heterotopic ossification, which is an ideal approach for complex acetabular fracture. Reconstruction plate is the first choice of internal fixator for complex acetabular fracture.
