56例多排螺旋CTA下肢动脉血管成像分析 被引量:8

摘要 目的:探讨多排螺旋CTA血管成像在下肢动脉血管疾病中的价值。方法:搜集了56例双下肢动脉多排螺旋CTA血管成像,正常6例,双下肢动脉粥样硬化8例、腘窝假性动脉瘤4例、双下肢大动脉炎10例、糖尿病性血管炎12例、股动脉血栓形成15例、股动脉搭桥1例,所有病例经手术或DSA证实。造影剂用量按2ml/kg,注射速率3ml/s。扫描延迟时间用testBolus,层厚2 ̄3mm。数据重建用最大密度投影(MIP)和实时三维(realtime3D)及容积显示(Fly3D)等。结果:6例为正常双下肢动脉血管,8例动脉粥样硬化性病变见不同程度的动脉壁钙化斑。4例腘动脉瘤CTA显示病灶的部位、大小、瘤蒂。10例大动脉炎见双下肢动脉血管管壁呈锯齿状,部分节段见血栓形成。12例糖尿病性血管炎与大动脉炎表现相近似。15例股动脉血栓栓塞,CTA见股动脉充盈缺损至管腔填塞,最长栓塞达60cm。1例人造血管CTA可清晰显示走向和双股动脉。结论:下肢动脉CT血管成像是一种安全、准确、无创的检查方法。 Objective: To detect the value of muhi-slice.spiral CT angiography (CTA) in lower extremity artery. Methods: Fiftysix cases of multi-slice spiral CT angiography (CTA) in lower extremity artery were analyzed, including 6 cases of normalities,8 lower extremity artery sclerosis, 4 popliteal false aneurysm, 10 arteritis of lower extremity artery, 12 diabetic arteritis, 15 thrombus of lower extremity artery, 1 by-pass artificial vascular in femur artery. All patients were confirmed by operation or DSA. The does of contrast agent was 2 ml/kg,and the rate of injection was 3 ml/s. Test bolus was used in delay time. Data reconstruction was showed by Real time 3D and Fly 3D (Volume rendering VR),multiple planar reformation (MPR),eurved planar reformation (CPR) and visual endoscopy (VE) in working-station. Results: Six cases presented with normalities, 8 cases of arterial sclerosis with calcification spot in the arterial wall. The site, size and pedicel of 4 cases of popliteal false aneurysm were showed. Ten cases of arteritis of lower extremity presented with lacerate wall in artery, 12 cases of diabetic arteritis with the same images, 15 cases of thrombosis of lower extremity artery with stenosis and obstruction,and the longest thrombus was 60 cm. Conclusion: Multi-slice spiral CTA in lower extremity artery is a safe, no-trauma and effective method.
