摘要
为探讨眼内肿瘤细胞能否种植于经细针穿刺活检后的巩膜穿刺道内,我们将Y79Rb细胞注入已用透明质酸酶液化玻璃体的32只兔眼内,造成不同眼压状态下Rb细胞眼内播散的动物模型,然后用5号针头自睫状体平部刺入球内,吸取少许玻璃体行细胞学检查,再将眼球摘除对巩膜穿刺道行常规病理检查,结果28个完整穿刺道切片中均未见有Rb细胞种植、嵌顿,巩膜纤维组织断端向外反折,形成自身嵌闭,其中高眼压组这种改变更为明显。实验病理证实细针穿刺活检诊断方法是可行的。
The purpose of this paper is to investigate the possibility of retinoblastoma cells implanting into the orbitthrough the scleral hole when the Fine Needle Aspiration Biopsy was used during the different clinical stagesof retinoblastomas for its differential diagnosis. 32 eyes from 16 rabbits were used for the experiment. Afterthe vitreous being hydrolysed by intraocular injecting hyaluronidium (Antidase), different amount ofretinoblastoma cells from Y-79 cell line were injected into eyes through the pars plana sclera to form twotypes of intraocular tumor cell desseminating animal models with different intraocular pressures (16 eyes foreach group) to imitate the human clinical intraocular and glaucomatous stages of Rb. Then FNABs were doneby using # 4-5 or # 5 needles through the pars planas and the cytology study was processed. Enucleationswere performed after 1 hour and routine pathology examination showed the needle penetration passage (hole)of the scleras. There was no tumor cells implanted or incorporated into the scleral holes from 28/32 relativelycomplete specimen. The scleral fibrous tissue ruptures and the broken ends protruded outward and formed thebarriers to block the wounds. This phenomenon was more obvious in the high IOP group. The result showsthat the FNAB technique is safe, reliable and could be applied in clinical practice.
