摘要
目的 探讨糖尿病视网膜病变与血脂的关系。方法 随机选择2型糖尿病患者90例,按有无糖尿病视网膜病变分组,测定他们的空腹血糖、血脂、脂蛋白、糖基化血红蛋白并进行分析总结。结果 DR组与NDR组比较血清总胆固醇(Tch)、甘油三脂(Tg)、低密度脂蛋白(LDL-C)、载脂蛋白B(APOB)明显增高(P<0.001);血高密度脂蛋白(HDL-C)、载脂蛋白A_1(APOA_1)明显下降(P<0.001)。PDR组与BDR组比较,Tch、Tg显著增高(P<0.001),而HDL-C显著下降(P<0.05)、APOA_1显著下降(P<0.001)。相关分析表明,DR患者的病变严重程度与FPG、HbA_1c、Tch、Tg、LDL-C及APOB呈显著正相关,与HDL-C及APOA_1呈显著负相关,结论 血脂代谢异常影响着DR的发生发展。因此,糖尿病患者应严格控制血糖、血脂水平。
Objective To observe the relationship of diabetic retinopathy with serum lipid. Methods Serum levels of lipids lipoproteins HbAle of 90 patients with type 2 diabetes mellitus were measured and analyzed. Results The total cholesterol(rich), triglyceride ( rig ), low - density lipoprotein ( LDC - C ) and Apolipoprtein B ( APOB ) were significandy increased ( P 〈 0. 001 ) in diabetic retinopathy patients group, while high - density lipoprtein ( HDL - C) and apolipoprtein A1 ( APOA1 ) were significantly decreased ( P 〈 0.001 ) in diabetic retinopathy patients group. The total cholesterol(rich) and triglyeeride (Tg) were significantly increased while high -density lipoprtein (HDL - C) and apolipoprtrein A1 (APOA1 ) were significantly decreased with proliferative diabetic retinopathy (PDR) than those with background DR (BDR). The correlation analysis showed the severity of DR was positively correlated with FBG, HbAlc, Tch, Tg LDL -C, APOB and duration of NIDDM and was negatively correlated with HDL - C and APOA1. Conclusion Abnormal metabolism of serum lipid influence the development and progression of DR . Thus strict control of serum lipid and the level of bloed sugar are equally important in diabetic patients.
