摘要
应用经颅多普勒超声(TCD)技术,观察36例普通型和典型偏头痛患者实施星状神经节阻滞(SGB)治疗前后大脑中动脉和颞浅动脉血流速度(υmca,vsta)的变化。结果发现,实施SGB前,36例患者的υmca和υsta较正常为高(P<0.05)。行SGB后,31例(86.1%)患者υmca和υsta下降接近正常,头痛消失或症状缓解;5例(13.9%)患者的颅内和颅外血流速度变化差异不显著(P>0.05),头痛症状无明显改善。提示偏头痛患者发作期的大脑中动脉和颞浅动脉主干处于轻度痉挛状态。多数患者对SGB和神经调节反应敏感,受累血管可能处于功能性轻度狭窄;而少数患者对SGB和神经调节反应较差,可能系该血管已有器质性改变或有其它潜在的致病因素。
Thirty-six migraine patients treated with stellate ganglion block(SGB)were studied with transcranial boppler(TCD).The changes of the velocity of the intracranial and extracranial blood flow before and after the treatment were compared in order to know the physiological change of the blood vessel of the brain.It was found that the blood flow velocity of the internal and external carotid artery were high before SGB.It indicated that the trunks of the middle cerebral artery and superficial temporal artery were in a state of spasm.After SGB,blood vessel spasm remitted or returned to a normal state;at the same time,migraine remitted or disappeared in most of the cases.It indicated that intracranial and extracranial arteries were sensitive to neural regulation, and the artery diameter was close to physiological state. In a few cases,the blood flow velocity had not changed,and the clinical symptoms had not remitted.It indicated that the intracranial and extracranial arteries were hyporeactive to neural regulation.There might be abnormality of pathomorphology in these blood vessels.
