影响颊黏膜鳞癌复发因素分析 被引量:1

摘要 目的探讨影响颊黏膜鳞癌复发的因素。方法分析颊黏膜鳞癌50例,以TNM分类、临床分期、分化程度、浸润深度、淋巴细胞浸润程度、微血管数、核分裂数等指标分析与颊黏膜鳞癌复发有关的因素。结果随着T值的增大、区域淋巴结转移、临床分期增高、分化程度变差,颊黏膜鳞癌复发人数增加;淋巴细胞浸润程度与复发无关。复发组浸润深度为5.90 mm,高于未复发组的4.82 mm。复发组的核分裂数(2.61个/HP)、微血管数(2.81条/HP)与未复发组的相应指标间差异无显著性。Logistic回归分析显示,各指标中只有临床分期进入回归方程,回归方程为Logit(p)=-3.887+1.224X(临床分期)。经Cox模型分析,证实复发是影响颊黏膜鳞癌预后的独立因素,其相对危险度为363.992,回归系数为5.897。结论①与颊黏膜鳞癌复发有关的因素有T分类、区域淋巴结转移、临床分期、分化程度、浸润深度;无关的因素有微血管数、核分裂数和淋巴细胞浸润程度。②临床分期是影响复发的独立因素。③复发是影响颊黏膜鳞癌预后的独立因素。 Objective To study the relationships between the clinical pathological characters and the recurrence of squamous cell carcinoma of buccal mucosa. Methods 50 cases were analyzed by means of the following factors: TNM classification, clinical classification, differentiation, tuner invasion thickness, invasion degree of lymph cell, and the number of capillary and pathological karyokinesis. Results ①The cases of recurrence were increased apparently with higher T classification, cervical lymph node spreading, higher clinical classification and poor differentiation. However the invasion degree of lymph cell was not related to recurrence. ②The tumor invasion thickness in recurrence group ( 5.90 mm) was higher than that in cured group (4.82 mm) ( t = 2.369, P 〈 0.05), while the number of pathological karyokinesis and capillary showed no statistic differences, compared with those of cured group, ③With Logistic regression analysis, clinical classification was the only factor which went into the equation: Logic (p) = - 3.887 + 1.224X(clinical classification). ④With Cox regression analysis, recurrence was the only independent factor related to the survival. Conclusion ①The factors that are related to the recurrence are T classification, cervical lymph node spreading, clinical classification, differentiation and the tumor invasion thickness. The number of pathological karyokinesis, the number of capillary and the degree of lymph cell invasion are not related to the recurrence. ②With regression analysis, clinical classification is the only factor that is related to the recurrence. ③Recurrence is the only independent factor that is related to the survival.
