摘要
目的:了解新型农村合作医疗可持续发展影响因素的现况和变迁。方法:通过领导意向调查,对浙江省试点实施新型农村合作医疗的地区政府部门(卫生与非卫生)和卫生技术人员针对下列潜在因素进行意向调查: (1)政府的重视程度和相应政策的权威性不够;(2)政府部门间围绕统一目标的工作难以协调;(3)政策压力欠缺, 实施随意性大;(4)宏观政策具体操作时,存在相当多的技术难点;(5)政府筹资过低;(6)未能针对农村居民的迫切需求;(7)群众集资多少的平衡点问题没有解决。结果:上述问题已得到不同程度的重视和改善,如政府的重视程度得分由原来的5．13上升到7．64:但同时其中有些问题还是依然存在,如:筹资问题、技术问题等等。同时,随着新型农村合作医疗工作的政府重视,技术层面上解决宏观政策的可操作性问题的重要性明显提高;通过新型农村合作医疗试点的实施,也提高了人群对上述诸因素可能影响新型农村合作医疗可持续发展的认识。
Objective： To find out the current status and the historical changes about the factors which may affect the sustainable development of New-type Rural Cooperative Medical System （NRCMS） . Method： The leader opinion survey method was employed to get the opinions of the leaders of the local health bureaus and hospitals in Zhe Jiang province. The survey content include： （1） absence of enough government attention and powerful relevant policies; （2） with the absence of uniform goal among the governmental department, it was difficult to harmonize their action; （3） with the absence of policy compulsion, the New-type Rural CMS was implemented optionally; （4） there still had lots of technical difficulties to actualize the macro-policy; （5） financing level was too low; （6） not aimed directly to resolve the rural citizen＇ most urgent demand; （7） the balance of private funding was unclear. Results： The survey results reveals that the upper factors have been paid more attention and improved at different levels. For example, the evaluating score of first factor rise from the formal 5.13 to 7.64; but some factors are still existing： such as funding problem, technical difficulties…; as the governmental attention improved, the understanding about the importance of how to actualize the macro-policy at technological level has risen obviously. Through implementing the pilot plan of the NRCMS, the understanding of the upper factor＇s affection in the NR CMS sustainable development has improved from the formal 61.90% to 84.30% at present.
