摘要
目的研究HLA-DRB1等位基因与HBV感染慢性化的相关性。方法用PCR-SSP方法对陕西地区汉族人群中的慢性乙型肝炎(CHB)患者54例、乙型肝炎表面抗原携带者(ASC)32例及正常健康人108人进行HLA-DRB1等位基因分型比较,并对HLA-DRB1等位基因分型与HBV不同复制状态进行相关性分析。结果陕西地区正常汉族人HLA-DRB1等位基因以DRB1*04(16·2%),DRB1*09(12·5%),DRB1*12(11·6%),DRB1*15(13·4%)最为常见;CHB组HLA-DRB1*03的等位基因频率(11·1%)明显高于健康对照组(3·7%),两组相比差异显著(Pc=0·014;OR=3·57;P<0·05);DRB1*07基因位点在HBV高复制状态较低复制状态多见(Pc=0·034,OR=3·88,P<0·05);HLA-DRB1*15等位基因频率对照组为13·4%,明显高于CHB组7·4%(Pc=0·039;OR=0·48;P<0·05)。结论HLA-DRB1*03、HLA-DRB1*07与陕西地区汉族人HBV感染后的慢性化相关联,HLA-DRB1*15在陕西地区汉族人与HBV感染的保护相关;HLA-Ⅱ类基因是决定HBV感染后临床转归的重要因素之一。
Objective To study the HLA-DRB1 genotype and their relation with HBV infection among Han patients in shanxi area, Methods HLA-DRB1 genotyping was conducted in 54 patients HBV infection and 108 health controls, as well as 32 asymtornatic HBsAg carriers by using polymerase chain reaction/sequence specific primer (PCR-SSP) method. All the patients, asymtornatic HBsAg carriers, and healthy subjects were inhabitanls of Shan'xi area of Han nationality. The association between HLA-DRB1 genotype and different replication of HBV were also studied. Results DRB1*04,DRB1*09,DRB1*12, DRB1*15 were the most common genotypes in the Shan'xi Han inbabitants with the frequency of 16.2%, 12.5%, 11.6% and 13.4% respectively. Compared to 108 healthy controls, the allele frequency of HLA-DRB1*03 was 11.1% in HBV patients versus 3.7% in healthy controls, with odds rati=3.57 and Pc=0.014 (P〈0.05). The allele frequency of DRB1*07 was most common in high replication group(Pc=0.034,odds ratio=3.88; P〈0.05). The allele frequency of HLA-DRB1*15 was 13.4% in HBV patients versus 5.6% in healthy controls, Pc=0.036; odds ratio=0.38 (P〈0.05) was obviously lower than HBV patients. Conclusions HLA-DRB1*03 and HLA-DRB1*07 are closely related with susceptibility to chronic hepatitis B, and DRB1*15 is closely related with resistance to chronic hepatitis B, These findings suggest that host HLA class Ⅱ gene is an important factor in determining the outcome of HBV infection.
