摘要
目的了解上海市白内障复明手术的现状。方法对2003年上海市95所医院施行的23 502眼白内障手术的情况进行分析。2004年4月在10个区中随机抽取2003年间施行白内障手术的患者371例548眼,进行裸眼视力、矫正视力、裂隙灯、眼底镜等检查。结果①23 502眼中脱盲22 829眼(97.14%),脱残19 560眼(83.23%),人工晶状体植入22 473眼(95.62%)。②548眼中脱盲527眼(96.17%)、脱残472眼(86.13%)、人工晶状体植入542眼(98.91%)、超声乳化加人工晶状体植入433眼(79.01%)。术后矫正视力<0.3的76眼中,高度近视眼黄斑变性31眼(40.79%)、糖尿病视网膜病变13眼(17.11%)、青光眼视神经萎缩8眼(10.53%)、手术并发症6眼(7.89%)、后发性白内障3眼(3.95%)、老年性黄斑变性3眼(3.95%)、视网膜脱离3眼(3.95%)、其他9眼(11.84%);86眼(15.69%)存在不同程度的并发症;168眼(30.66%)散光度数≥±2.00 D;术后矫正视力≥0.5者446眼(81.39%),≥1.0者98眼(17.88%)。结论上海市白内障复明手术质量欠理想,手术质量有待进一步提高。
Objective To learn the current status of cataract surgery for treating “blindness” in Shanghai. Methods In 2003, the results of 23 502 eyes from 95 hospitals in Shanghai were analysed after cataract surgery. In April 2004, 548 eyes （371 patients） from 10 districts with cataract surgery in 2003 were sampled randomly and tested by naked visual acuity, best corrected visual acuity, slitlamp and funduscopic examinations,etc. Results （1） Of 23 502 eyes, 22 829 eyes（97. 14%） got rid of blindness. 19 560 eyes （83.23%） got rid of handicap. 22 473 eyes （95. 62%） were implanted with intraocular lens. （2） Of the above 548 eyes sampled randomly, 527 eyes （96. 17%） got rid of blindness,and 472 eyes （86.13%） got rid of handicap. 542 eyes of them（98.91%） had intraocular lens implantation. 433 eyes （79.01 % ） underwent phacoemulsification and intraocular lens implantation. After operation, 31 （40.79~） of 76 eyes with corrected visual acuity not less than 0.3 had macular degeneration and high myopia. 13 eyes（ 17.11 ~） had diabetic retinopathy. 8 eyes（ 10.53 %） had optic atrophy from glaucoma, the reasons of remaining blindness after surgery were surgical complications in 6 eyes （7. 89%）, postoperation cataract in 3 eyes （3.95%）, age-related macular degeneration in 3 eyes （3.95%）, retinal detachment in 3 eyes and others in 9 eyes （11.48%）. 86 eyes （15. 69%） had various complications. The degree of the astigmatism was no less than ±2.00 D in 168 eyes （30. 66%）. 446 eyes （81.39%） had corrected visual acuity of no less than 0. 5 and 98 eyes （17.88%） no less than 1.0. Conclusions The current status after cataract surgery for “blindness” has not been ideal in Shanghai, the skillful operability has to be promoted further. （Shanghai Med J,2005, 28：776-778）
