肝气郁结证相关脑区的正电子发射断层成像 被引量:10

摘要 目的:观察肝气郁结证整体水平上脑功能变化的特点,获得肝气郁结证中枢病变的定位信息。方法:采用配对研究的原则,应用正电子发射断层扫描成像技术,选择典型肝气郁结证患者60例,应用SPM图像分析软件,对比分析肝气郁结证与正常志愿者脑功能影像学的不同。结果:左侧额下回(BA47)、扣带回前部、扣带回后部、颞中回(BA21)、额中回(BA6)、双侧颞上回(BA37,38)、中央前回(BA4,6)等表现为葡萄糖低代谢状态(P<0.01);脑干中脑、延桥交界、右侧楔叶(BA18)、左侧小脑表现为葡萄糖高代谢状态(P<0.01)。结论:肝气郁结证相关脑区多呈现葡萄糖利用率降低,整体水平上为中枢参与肝气郁结证病变提供了客观的可视性影像学依据。 Objective: To Observe the character on brain function change of syndrome of stagnation of liver qi, and get the location information of central pathological changes. Method: Adopting pairing research principle, applying positron emission tomography (PET), selecting typical patients with syndrome of stagnation of liver qi, utilizing SPM image analysis software, the difference of brain function imageology between patients with syndrome of stagnation of liver qi and normal volunteers were contrasted and analyzed. Result: Left inferior frontal gyrus (Brodmann area 6), cingulate gyrus anterior part, cingulate gyrus posterior part, middle temporal gyrus (Brondann area 4, 6), middle frontal gyrus (Brondann area 6), hibateral gyrus temporalis superior (Brondann area 37,38) and precentral gyrus (Brondann area 4,6) etc. show hypometabolism state of glucose (p<0.01). Brain stem midbrain, borderline of medulla oblongata and pons cerebelli, right cuneus (Brodmann area 18) and left cerebellum show hypermetabolism state of glucose (p<0.01). Conclusion: Related encephalic regions of syndrome of stagnation of liver qi mostly present lowly glucose utilization rate. That provides objective visibility imageology basis for center participating pathological change of syndrome of stagnation of liver qi from integrated level.
