摘要
目的探讨急性心肌梗死(AM I)患者血浆同型半胱氨酸(Hcy)水平与纤溶活性,内皮细胞功能,炎症因子的关系及其临床意义。方法分别测定AM I患者(162例)及健康对照组(90例)血中Hcy水平以及纤溶酶原激活抑制物-1(PAI-1)活性,血管性血友病因子(vWF),心肌肌钙蛋白I(cTnI),D-二聚体(DD),C反应蛋白(CRP)及血脂各项指标,用统计学方法分析Hcy与它们的关系。结果AM I组Hcy水平较对照组明显升高(P<0.01),AM I患者PAI-1活性增强,vWF及DD水平明显高于对照组(P<0.01),CRP及CTnI在AM I组呈阳性反应。多因素相关性分析结果显示Hcy与PAI-I、vWF、CRP、cTnI、DD均有良好相关性,但与血脂无关。结论Hcy是AM I独立危险因素。Hcy可能通过损伤血管内皮细胞,引起血管壁炎症反应,抑制纤溶系统,促进AM I的发生、发展。
Objective To investigate the correlation between plasma homocysteine level and function of endothelial cell,fibrinolytic activity,inflammation substances in acute myocardial infarction (AMI) and its clinical significance. Methods The plasma total levels of homocysteine (Hcy) ,plasminogen activator inhibitor- Ⅰ (PAI- Ⅰ ), von willebrand factor (vWF) ,cardiac troponin Ⅰ (CTnI), D-dimer (DD), C-reaction protein (CRP), triglyceride (TG), total cholesterol (TC) of 162 cases with AMI and 90 healthy controls were detected respectively. Results Hcy,vWF,PAI- Ⅰ and DD level in AMI group were significantly higher than those in control group ( P 〈 0.01 ). There was a action reaction of CRP and CTnI in AMI group. The multi-factors related analysis showed that Hcy had significant correlation with CRP,CTnI, vWF,PAI- Ⅰ and DD,but had no significant correlation with TG. Conclusions Hcy is an independent risk factor for AMI,which might lead to AMI through injuring vessel endothelial cells,causing inflammation,and inhibiting fibrinolytic activity.
