摘要
[目的]探讨Link胫骨上段解剖钢板固定治疗胫骨平台骨折的手术方法及疗效评估.[方法]本院1996年1月～2004年7月住院患者128例,按胥少汀分类,Ⅱ型92例,Ⅲ型36例.均经手术治疗行单侧骨膜剥离,对平台骨折并塌陷者,在保持关节面平整基础上行坚强内固定并植骨,早期膝关节功能锻炼.[结果]所有病例均获6个月～18个月的随访,全部病例均骨性愈合,愈合时间最短为8周,最长为14周,平均为12周,膝关节功能评价,优108,占84.38%,良16例,占12.50%,可4例,占3.12%,优良率为96.88%.[结论]Link钢板加植骨治疗胫骨平台骨折,对加快骨折愈合,防止胫骨平台塌陷,便于膝关节早期功能锻炼,预防关节僵直和创伤性关节炎有显著疗效.
[ Objective] To investigate treatment and clinical results of tibial plateau fractures with link plate and bone autograft. [ Method] A total of 128 cases of tibial plateau fractures were treated with internal fixation of link plate and bone autograft from January, 1996 to July ,2004. According to LUYUPU classification, there were 92 cases of type Ⅱ, 36 cases of type Ⅲ. All of the cases were treated with open reduction. Result：All 128 cases were followed up from six months to eighteen months. According to Hohl＇s criteria, the result were： 108 cases being excellent （84.38%） , 16 cases being good （12.50%） and 4 being fair （3.12%）. The satisfactory rate was 96.88%. [ Conclusion] It is effective to use Link plate and hone autograft to treat the cave-in of the tibial plateau fractures. It can accelerate the healing procedures, prevent ankylosis and traumatic arthritis. Functional exercise can he started at early stage. It provides rigid fixation for the fracture with low risk of soft tissue complications.
