脑电图与视觉诱发电位对抽动秽语综合征患儿注意力焕散现象的评估 被引量:2

Electroencephalogram and visual evoked potential in evaluating aprosexia in children with Tourette syndrome

摘要 目的:探讨脑电图和视觉诱发电位对抽动秽语综合征患者注意力焕散现象的评估作用。方法:选择2002-03/2005-01德州市人民医院门诊诊治抽动秽语综合征患者100例为患者组,男68例,女32例;年龄4~12岁;诊断标准参照DSW—IV关于抽动障碍的分类及诊断标准。随机抽取正常健康查体儿童96例作为对照组,男64例,女32例;年龄4~14岁;无躯体及精神疾患,既往身体健康。在闭目安静状态下,常规脑电图描记,部分病例做闪光刺激及蝶骨电极诱发试验。视觉诱发电位使用肌电诱发电位仪,在20~25℃安静屏蔽室内检查,检查前3d内禁服中枢镇静药和抗精神病药物。结果:病例组100例,脑电图呈正常及边缘状态50例,轻度异常32例,中度异常18例,无重度异常,总异常率50%(50/100)。对照组92例,脑电图呈正常及边缘状态92例,轻度异常4例,无中度异常及重度异常,总异常率4.17%(4/96)。病例组与对照组脑电图异常程度差异有显著性意义(χ2=51.9988,P<0.001)。病例组视觉诱发电位异常率46%,其中单侧异常18例,双侧异常28例,病例组P100潜伏期及N145峰潜伏期比对照组明显延长,差异有显著性意义犤P100潜伏期(101.62±7.50),(96.80±3.98)ms;N145峰潜伏期(164.67±33.14),(152.67±16.65)ms,t=80.33,45.48,P<0.001犦。结论:脑电图异常结果提示抽动秽语综合征患儿有一定的脑功能障碍,视觉诱发电位P100,N145潜伏期及峰潜伏期延长说明视觉皮质中枢受到一定损害,从而出现注意力涣散现象。 AIM: To investigate the role of electroencephalogram and visual evoked potential.in evaluating aprosexia in children with Tourette syndrome. METHODS: Totally 100 children with Tourette syndrome (68 males and 32 females) at 4 to 12 years of age, who were treated in the out-patient clinic of Dezhou People's Hospital between March 2002 and January 2005, were selected as the patient group; The diagnostic criteria referred to the classification and diagnostic standard of tic disorder in Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (DSM-Ⅳ); and 96 children (64 males and 32 females) at 4 to 14 years of age, who were the normal physical examinees and had no somatic and mental diseases, were randomly selected as controls (control group). At rest with closed eyes, regular electroencephalogram tracing was undertaken, part of the patients received flash stimulation and sphenoidal electrode evoked tests; Electromyographic evoked potential meter was used for examination of visual evoked potential at 20 to 25 ℃ in a quiet shield room, and they were forbidden to taken any drug for central sedation and antipsychotic drags within the 3 days before examination. RESULTS: Of the 100 cases in the patient group, electroencephalogram was normal or at borderline state in 50 cases, mildly abnormal in 32 cases, moderately abnormal in 18 cases, no severely abnormal was observed, and the total abnormal rate was 50% (50/100); Among the 96 cases in the control group, electroencephalogram was normal or at borderline state in 92 cases, mildly abnormal in 4 cases, no moderately or severely abnormal was observed, and the total abnormal rate was 4.17% (4/96); There was significant difference in the abnormal degree of electroencephalogram between the patient group and control group (X^2=51.9988,P 〈 0.001). The abnormal rate of visual evoked potential in the patient group was 46%, including 18 cases had unilateral abnormality and 28 cases had bilateral abnormality; The latent periods of P100 and N145 peak was significantly prolonged in the patient group than in the control group [latent period of P100: (101.62±7.50), (96.80±3.98) ms; latent period of N145 peak: (164.67±33.14), (152.67±16.65) ms, t=80.33, 45.48, P 〈0.001]. CONCLUSION: The results of abnormal electroencephalogram indicate that children with Tourette syndrome have certain disorder of brain function, the prolonged latent period and peak latent period of visual evoked potential P100 and N145 shows that visual cortical center is damaged and then leads to aprosexia.
