摘要
目的探讨以尿α1微球蛋白(α1M)作为评价新生儿缺氧缺血性脑病(HIE)患儿肾功能损害指标的价值及尿α1M的水平与脑组织损伤程度之间的关系。方法设研究组37例HIE患儿及对照组9名正常足月儿,采用酶联免疫吸附法(ELISA)及全自动生化分析仪检测新生儿第2~3天的血清中的α1M、血尿素氮、血肌酐及尿α1M的浓度,结合相关临床资料对比分析,并进行统计学处理。结果新生儿HIE组尿α1M明显高于正常对照组(P<0.05);轻度、中度、重度HIE患儿尿α1M相互之间进行比较,结果提示轻度与中度、重度HIE患儿尿α1M差异均存在统计学意义(P<0.05);中度、重度HIE患儿尿α1M之间差异无统计学意义(P>0.05);轻度HIE患儿与正常足月儿之间尿α1M差异无统计学意义(P>0.05)。经相关分析,尿α1M与HIE患儿的轻重程度存在相关性,r=0.556,P<0.01。HIE患儿三组间血清α1M差异无统计学意义(P>0.05)。结论尿α1M的测定可以早期判定HIE患儿的肾功能损伤;尿α1M与HIE的分度呈正相关,对协助临床分度、指导治疗及估计预后有一定价值。
Objective To probe into urine level of α14-microgldmlin as an evaluation index to the renal function impairment of newborns with hypoxic-ischmaic encephalopathy (HIE) ,and its relationship to the injury degree of brain tissue. Methods α1-Microglobulin in senam,blood urea nitrogen,blood creatinine and α1-microglobulin in urine of HIE(37) and normal control (9) were measured with enzyme-linked inmatlnosorbent asseay and whole automatic biochemistry analyzer, dealing with some data by means of statistic method. Results The urine level of α1-microglobulin in HIE was significantly higher than that in normal control (P〈0.05).Among different stages of HIE, the urine level of α1microglobulin of infants with stage 2 or stage 3 was significantly higher than that with stage 1 (P〈0.05). The difference of urine levels of α1-microglobulin between stage 2 and stage 3 was not significant (P〉0.05).At the same time the difference of urine level of α1-microglobulin between stage 1 and normal control was not significant (P 〉 0.05). Through correlation analysis, the urine level of α1-microglobulin was assodated with the stage of HIE (P〈 0.01, r =0.556). At different stages of HIE,the difference of the serum levels of α1-microglobutin was not significant. Conclusion Urine level of α1-microglobulin may be uese for evaluating the renal function; it has positive corrdation with the stage of HIE, therefore it may have great value on earlier assessing the stage of HIE, instructing the treatrnent and appraising the prognosis.
