摘要
目的探讨用手套阻止乙型肝炎病毒(HBV)经手术传播的效果. 方法在确定手术中HBV传染性和感染阈值的基础上,探讨用手套阻止HBV经手术传播的效果. 结果 HBV经过单层手套感染的阈值为105 ID/ml;经双层手套感染的阈值为106 ID/ml;HBsAg阳性、HBsAg阴性、HBeAg阳性和HBeAg阴性患者的传染性分别是0～109、0～104、102～109和0～106 ID/ml;分别有19.2%、0、83.9%和3.2%患者的传染性超过单层手套感染阈值,为手术中的传染源;76.7%的HBsAg阳性和80.8%的HBeAg阳性传染源的传染性超过双层手套的感染阈值,最大传染性为103 ID/ml;HBeAg阴性传染源有50.0%达到双层手套的感染阈值,但传染性仅为临界水平.结论在手术中,单层手套只能消除HBsAg阴性血液的传染性,双层手套虽能阻止HBeAg阴性血液传染,但对大部分HBeAg阳性血液防护无效.
OBJECTIVE To evaluate the effect of that the gloves would prevent hepatitis B virus （HBV） from infection during surgical operation. METHODS On the basis of that HBV infectivity and infective threshold value were determined during surgical operation, evaluate the effect of that the gloves would prevent HBV from infecting during surgical operation. RESULTS HBV infective threshold value of single layer gloves was 10^5 ID/ml, and that of double layer gloves was 10^6 ID/ml. The infectivity of HBsAg positive, HBsAg negative, HBeAg positive and HBeAg negative patients was 0-10^9 , 0-10^4 , 10^2-10^9 and 0-10^6 ID/ml, respectively. The rates of patients whose infectivity exceeded HBV infective threshold value of single layer gloves were 19. 2% , 0, 83. 9% and 3.2% , respectively. The HBV infective source was existed during surgical operation. The infectivity of 76.7% HBsAg positive and 80. 8% HBeAg positive infective source exceeded HBV infective threshold value of double layer gloves. The maximum infectivity of them was 10^3 ID/ml. The infectivity of 50.0% HBeAg negative infective source reached HBV infective threshold value of double layer gloves. But it was barely a critical level. CONCLUSIONS During surgical operation, the infectivity of HBsAg negative blood can be eliminated by using single layer gloves. The double layer gloves can prevent the infection from the HBeAg negative blood but they can＇ t prevent the infection from most part of HBeAg positive blood.
