摘要
为考核中国精神疾病分类和诊断标准第2版修订本(CCMD-2-R)的性能与临床推广的可行性,作者于1993年10月至1994年3月进行了现场测试。全国有18个单位的55位精神科医师参加,共测试了750例。87.1%认为文本的可理解性好,易于接受;84.0%认为可操作性好;64.6%认为CCMD-2-R 比 CCMD-2有所进步。CCMD-2-R 标准诊断与临床诊断的一致率为93.6%(K=0.80),与 CCMD-2诊断的一致率为96.5%(K=0.89)。评定员间诊断的首位编码的一致率为97.4%(K=0.97),4位编码的一致率为84.9%。92.5%的 CCMD-2-R 诊断肯定,适合具体病例者达91.5%。结论:CCMD-2-R 具有良好的信度、效度和适用性,宜于临床推广应用。
The national field trials of the revision of the 2nd edition of the Chinese classification and diagnostic criteria of mental disorders (CCMD-2-R),involing 18 psychiatric institutes and 55 profes- sionals,were conducted from October 1993 to March 1994.Most researchers (87.1%)considered its comprehensibility and acceptability good and 84.0% considered it applicable.Comparing with CCMD-2, 64.6% reported it somewhat better.Altogether 750 eases were tested.The consistency rate between the CCMD-2-R diagnoses and the clinical diagnoses was 93.6% (Kappa= 0.80,P<0.01).Interrater consistency was 97.4% for the first digits of coding of the diagnoses and 84.9% for the all four digits of the diagnoses.The majority (91.5%)of the CCMD-2-R diagnostic criteria provided a good adaptability for the clinical psychiatric conditions encountered and the diagnoses were suitably represented.The par- ticipants of the research drew a conclusion that CCMD-2-R was good in reliability,validity and applica- bility.
