联合吸入沙美特罗/丙酸氟替卡松对哮喘患者血小板膜蛋白分子表达的影响 被引量:1

摘要 目的研究联合吸入沙美特罗/丙酸氟替卡松(舒利迭)对哮喘患者血小板膜蛋白分子CD62P、CD40配体(CD40L)表达的影响。方法体外分离29例哮喘患者血小板,用流式细胞仪测定哮喘患者持续吸入沙美特罗/丙酸氟替卡松治疗前后血小板膜蛋白分子CD62P、CD40L的表达,并分析其与肺功能的关系。结果哮喘急性发作患者治疗前血小板膜蛋白分子CD62P、CD40L表达率为(46.7±12.8)%、(21.1±7.4)%,均较正常人(10.4±2.7)%、(4.4±2.1)%升高(P<0.01),且CD62P、CD40L的表达水平与患者的1秒钟用力呼气量(FEV1)占预计值百分比(FEV1%Pred)成负相关(r=-0.541,P<0.01;r=-0.506,P<0.01);持续吸入沙美特罗/丙酸氟替卡松3个月后CD62P、CD40L表达率为(11.8±4.7)%(、5.3±2.9)%,较治疗前明显下降(P<0.01)。结论哮喘急性发作患者存在血小板膜蛋白分子CD62P、CD40L的表达增高,其表达水平与哮喘患者的FEV1%Pred呈显著负相关,持续吸入沙美特罗/丙酸氟替卡松可以抑制其表达,血小板可能是沙美特罗/丙酸氟替卡松作用的又一种靶细胞。 Objective To investigate the effect of inhaled salmeterol/fluticasone propionate combination (seretide) on expression of platelet membrane protein molecules in patients with asthma. Methods Platelets were isolated from 29 patients with asthma, before and after inhaled salmeterol and fluticasone 3 months, the expression rates of platelet membrane protein molecules CD62P,CD40 ligand(CD40L) were measured by flow cytometry. Meanwhile the pulmonary function of all participants were evaluated, the relationship between pulmonary function and the expression of platelet membrane protein molecules was analysed. Results The expression of platelet membrane protein molecules CD62P,CD40L were (46.7±12.8) %,(21. 1±7.4) % in patients with acute asthma group,which were significantly increased as compared with control group( n = 15) ( P 〈 0.01 ), they were negatively correlated with their FEV1% in predicted value( r =-0. 541, P 〈0.01: r =-0. 506, P 〈0.01). After long-term inhaled salmeterol and fluticasone therapy,the expression of CD62P,CD40L were (11.8±4.7) % and (5.3±2.9) %,which were significantly declined compared with those before therapy( P 〈0.01). Conclusion There was high level expression of platelet membrane protein molecules CD62P, CD40L in patients with acute asthma, and the level was negatively correlated with its pulmonary function, long-term inhaled salmeterol and fluticasone could inhibit the expression of CD62P, CD40L. Perhaps platelet was another target cell of long-term inhaled salmeterol and fluticasone therapy for asthma.
