摘要
五年应用鱼口交锁髓内钉治疗股骨干骨折114例,全部随访1年以上,愈合率100%,优良率97.4%,该钉可防止断端短缩和旋转,适应症扩大到峡部以外和不稳定骨折,粉碎Ⅳ型为绝对适应证.该钉远端鱼口状槽简化锁钉难度,减少X线接触量.该钉动力性加压促进骨不连的骨性愈合,本组21例骨不连全部骨性愈合.该交锁钉的远期疗效与钉体合理设计、术前充分准备、术中严格操作、术后合理训练密切联系.
14 cases of femoral shaft fractures were treated by fish:-mouth-shaped interlocking intramedullary nails over a 5-year period. The duration of follow-up was more than one year. The union rate was 100%,and good results were obtained in 97. 4% of the cases. The nails could avoid limb shortening and torsional deformity. Fractures around strait and in unstable patterns were also its indications,especially in IV type comminuted fractures. The distal part of the nail was like fish-mouth,which could simplify the procedure of locking screws and reduce contact with X-ray. Dynamic compression of the nail was good to the union of fractures ,and all 21 non-u-nion cases in this series have been united with this approach. Long-term effects of the nails were associated with its ideal design,as well as good preparation before operation,accurate performance during operation,and suitable training after operation on the patients.
