摘要
目的应用立体定向放射(X刀)治疗脉络膜黑色瘤观察其近期疗效。方法脉络膜黑色素瘤患者7例7眼,全身未发现转移病灶。肿瘤基底最大径线平均11.58mm;基底最小径线平均9.36mm;高度平均11.13mm。采用SiemensPRIMUS直线加速器6MVX线单次放射治疗(X刀)。治疗后1d、1m、3m、6m、12m及随后每6m复查。如随访中发现肿瘤增大,则行眼球摘除术。结果所有患者随访9m～25m,平均16.9m。全部患者生存,未发生肿瘤复发和眼球摘除。所有病例裂隙灯眼前节检查无异常。CT测定术后肿瘤缩小率(19.51～67.06)%,多在20%～30%之间。彩色多普勒检查显示6例肿瘤血流减少。每6m全身检查未见肿瘤远处转移。术后2例患者出现放射性视网膜病变,未见干眼症、白内障、青光眼。结论平均16.9m随访结果表明X刀治疗脉络膜黑色素瘤有效,长期疗效需进一步观察。除CT检查,彩色多普勒亦可作为检测肿瘤大小和血流的有效检查。
Objective To study local tumor control and radiogenic side effects after stereotactic radiation therapy for uveal melanoma. Methods Seven patients （with seven eyes） diagnosised as uveal melanoma, 5 males and 2 females, were treated with stereotactic radiation therapy. The general examination indicated no distant tumor movement befroe treatment. The largest average base diameter was 11.58mm, the least aver age base diameter was 9.36mm, the average height was 11.13mm. After immobilization of head and eye, the tumors were treated by stereotactic radiation therapy, based on linear accelerator （6 MV） （Simense PRIMUS）. The patients were followed by 1 day, 1 month, 3 months, 6 months, 12 months and then every 6 months. The eye will be enucleated as soon as the tumor become larger in follow-up. Results The patients were followed up without distant movement 9～25 months, mean 16.9 months. The decrease of tumor was 19.51 ～67.06% （mostly 20～30 % ） by CT scan. Six tumors＇ blood fluid decreased detected by color doppler. There were 2 cases displayed radiogenic retinopahty. Conclusions During a follow-up period for 16.9 months, local tumor control was achieved in all eyes and a long term investigation was needed. Retrobubal anaesthesia method for stabilization of the eye was effective. Color doppler check was another use- ful technique beside CT scan.
