摘要
目的:观察4种大连接体设计形式对基牙及牙槽黏膜的力学影响。方法:应用三维有限元法,检测前腭板、后腭杆、全腭板以及前腭板加后腭杆4种大连接体义齿在受力时,基牙和牙槽黏膜应力分布及大小。结果:后腭杆设计的义齿使基牙受力最大,前腭板加后腭杆设计对缺牙区牙槽黏膜影响最大。结论:上颌双侧末端游离缺失病例,基牙条件较差者,不宜单独使用后腭杆形式的大连接体;缺牙区牙槽骨、黏膜条件较差者,应避免前腭板加后腭杆形式的大连接体。
Objective: Four types of major connectors were designed for bilateral distal-extension removable partial dentures. The purpose of this study was to evaluate the mechanical effect of each connector design on abutment tooth and alveolar mucosa. Methods: We chose four types of major connectors:anterior palatal plate, posterior palatal bar, full palatal plate and anterior palatal plate with posterior palatal bar. The FEA method was utilized to determine the stress strength and stress distributions on abutment tooth and alveolar mucosa when each major connector was loaded. Results: Posterior palatal bar maximized the load on abutment tooth. Anterior palatal plate with posterior palatal bar had greatest influence on alveolar mucosa. Conclusions: If the maxillary first premolars are not sound enough, we don' t recommend to use the posterior palatal bar solely as a major connector. Anterior palatal plate with posterior palatal bar can not be used on the patient whose alveolar mucosa is not healthy.
